Field Surveying: The existing available Lidar is detailed and was done when the river was fairly low, so FA will gather some additional bathymetry and miscellaneous information to tie everything together. Expected survey limits are shown in the figure below with the limits highlighted in red. This data combined with the Lidar will be used to create a topographic and right-of-way survey. Additional physical features within the impacted area along with right-of-way limits will be determined. The survey will incorporate visible above-ground evidence and underground utility locate mark outs, provided by Digline utility locators. The tasks required to map the limits of the topographic and boundary right-of-way ‘Design Surveys’ for the structures are detailed in the tasks below: 2.1. Right of Entry Notices: FA will provide “Right of Entry” notices per the requirements set forth in Idaho Code Title 54, Chapter 1230. It is assumed 14 properties will need to be notified. FA will post right of entry notifications on potentially affected properties near the project area prior to entering said properties. FA will address the phone calls from the landowners after their receipt of the right-of-entry notification letter. 2.2. Survey Mapping and Records Research: FA will conduct research of existing subdivision plat and record of surveys with the respective City and County as well as current maintained use to aid in determination of ownership limits. 2.3. Utility Locates and Historical Utility Mapping Collection: FA will request Digline mark outs for underground utilities and will mail an Initial Contact Letter to the utility companies listed in the dig-line facilities response requesting information regarding their utilities and locations. FA will mark the Digline limits for approximately 2500 foot on the north side of the river, and at the of the project on one occasion. FA cannot Detailed Scope of Work Teton River Connecting Path & Ped Bridge Key No. 24684 Page 4 of 22 guarantee the utility companies will respond or mark the location when it’s a design request. 2.4. Field Survey A. Survey Control: Establish survey control around the areas to be surveyed. We will use NAD 83 State Plane Grid East Zone (1103) horizontally and NAVD 88 (Geoid 18) vertically. The Archaeological and Historic Survey Resource Report (AHSR) Addendum will follow Idaho State Historic Preservation O)ice (SHPO) standards and format, and shall include background research, field survey, and preparation of the report addendum. The AHSR Addendum will include discussions of field methods, survey results, and maps showing areas surveyed. Consultant will provide su)icient information to prepare the Determinations of Significance and E)ect by SHPO. Project Coordination DEA cultural sta), at times, DEA cultural sta), at times, may need to coordinate with LHTAC, ITD and various other interested parties. Project-related questions will be directed to Forsgren. Once the project is under contract, DEA will contact ITD HQ Cultural Resources Sta) to discuss the applicable field techniques with the Highway Archaeologist and/or Architectural Historian to determine the appropriate methodology. QA/QC shall be done by DEA for the deliverables. ITD 1500 Form This task will be completed by LHTAC. Research Database checks are required for previous surveys and known cultural resources within one mile of the project area. The exact number and nature of all cultural resources within the inventory area will be confirmed with the records search conducted at the Idaho SHPO through their Idaho Cultural Resources Information System (ICRIS). Additional research may be conducted with the Idaho Historical Library, county historical society, and/or the county assessor. Appropriate General Land O)ice (GLO) and historic maps shall be inspected for any historic features that may be within the inventory area/APE as part of the background research. Subsurface Investigations Due to the nature of the project location in a relatively undeveloped location along the Teton River, subsurface archaeological deposits may be present. It is recommended that up to five subsurface test pits be completed for the project. Shovel testing will be conducted by hand with a shovel. All materials will be passed through 1/8 inch wire mesh screen and the contents will be replaced back into each test location. Fieldwork Applicable field investigation techniques will be discussed with Highway Archaeologist/Architectural Historian to determine the appropriate methodology. Project leaders will provide a full project description including measurements and figures that show all proposed areas of ground disturbing David Evans & Associates activities. This will include all proposed excavation, staging, material storage, and any other area that has potential for ground disturbance. The project will be surveyed using intensive survey methods using pedestrian transects spaced no more than 30 meters apart but may be spaced closer depending on the environmental conditions. The field survey will follow Idaho SHPO and Archaeological Survey of Idaho (ASI) guidelines. Private property owner information will be provided to DEA from Forsgren. Resource Records One cultural resource will be recorded under this scope of work. This resource will be fully documented to provide data on location, dimensions, content age, context, and integrity. The site will be recorded to current standards and evaluated for National Register of Historic Places (NRHP) eligibility following the guidelines in National Register Bulletin 15 and the Consulting with Idaho SHPO Handbook. Depending on the type of cultural resources identified, the resources will be evaluated according to Idaho SHPO and ASI guidelines, and the resource record will be produced through the ICRIS system. Cultural Resource Survey Report The AHSR will follow Idaho SHPO and ASI guidelines and will include research, field survey methods, survey results, findings (including site condition and eligibility), potential project e)ects, recommendations, maps, photos, and appropriate site forms. Resources, which are noted, but not considered sites will be discussed in the AHSR rather than recorded on separate site forms. All Noted but Not Recorded (NBNR) resources will be photographed, plotted on a map, and listed in a table within the report. DEA will address all comments, if any, received by LHTAC, ITD, and SHPO and revise and resubmit as needed. Technical Review QA/QC Prior to submitting any cultural resource documents, internal Quality Assurance and Quality Control shall be completed by DEA cultural resource sta). Comment Response The draft report will be submitted to Forsgren, LHTAC, and ITD HQ Cultural Resources Sta) for review. Comments will be addressed, and the revised report addendum will be resubmitted for review and concurrence. The final revised report addendum will be submitted to SHPO through ICRIS for review and concurrence. ITD and SHPO are allowed a 30-day review. Should any agency sta) require report edits or revisions, the review cycle for each is renewed at the time of the submitted report changes Assumptions • Waste sites, staging areas, and materials source sites will be provided to the consultant. • Ten subsurface test pits will be completed along the project. David Evans & Associates • Three cultural resources are anticipated to be recorded by DEA under this scope of work. If additional sites are identified, these will be recorded and evaluated under an additional supplemental agreement. • If NRHP-eligible sites are identified that would be adversely a)ected, a Determination of Adverse E)ect (E106), Memorandum of Agreement (MOA) and Section 4(f) documentation must be conducted to complete Section 106 compliance. If these documents are needed, they will be considered additional services. Mrytle Street, Suite 300 Boise, ID 83702 RE: PROPOSED SCOPE OF SERVICES TETON RIVER CONNECTING PATH & PED BRIDGE KEY NO. 24684 Dear Brent: UNDERSTANDING This project includes constructing a section of new pedestrian pathway along the north bank of the Teton River and construction of a new pedestrian bridge over the river. The bridge will connect to existing pedestrian pathway along the south bank of the river. The proposed bridge crossing will be located about 2,500 feet east of N 2nd E Road and about 800 feet north of Barney Dairy Road (Coordinates: 43.835480°,-111.768508°) in the City of Rexburg, Idaho. The proposed bridge span is currently unknown but is anticipated to be about 150 to 200 feet and include abutments and a possible center pier. Depending on depth to bedrock and estimated channel scour depth, it is assumed the bridge may be supported by rock socketed piles, drilled shafts, or shallow foundations. Additionally, minor retaining walls may be constructed for ramped up areas to the bridge or to restrict fill into adjacent right-of-way. The proposed pedestrian pathway will be about 10-feet-wide. The City has provided a standard section for the pathway and therefore, design of the pavement section is not required. The following information has been helpful for our understanding. • Our general knowledge and practice in the project vicinity • Conversations with Forsgren Associates • Imaging from Google Earth dated July 27, 2025 • Idaho Geological Survey information • Idaho Department of Water Resources Mr. Brent Schiller, PE Forsgren Associates July 24, 2026 Page 2 of 8 Project No. 71-116716 • U.S. Geological Survey • USGS and FEMA flood maps • Department of Agriculture Natural Resources Conservation Services (NRCS) DELIVERABLE Shannon & Wilson will submit the following:  Geotechnical Engineering Report providing bridge and retaining wall structure information in support of: - ITD Materials Manual - Section 230 relating to foundation and earth retaining conditions and recommendations. - Special provisions and contractor notes. - LPile analysis files  Final Design Review Letter to confirm the intent of the Geotechnical Report has been incorporated into the bid proposal documents. TECHNICAL APPROACH For structures, we will use methods outlined in the 10th edition LRFD AASHTO Bridge Design Specifications. Seismic values will be based on the 9th edition per ITD Bridge requirements. Our work will apply the requirements of the ITD Materials Manual. SCOPE OF SERVICES Shannon & Wilson proposes to provide materials and geotechnical services in accordance with a five-step process, consisting of: 1. Reconnaissance and Advance Preparations 2. Subsurface Explorations 3. Laboratory Testing 4. Data Development and Analyses 5. Reporting Specifically, we propose the following activities, which are included in Shannon & Wilson’s cost estimate work breakdown. Mr. Brent Schiller, PE Forsgren Associates July 24, 2026 Page 3 of 8 Project No. 71-116716 Reconnaissance and Advance Preparations  Acquire/review historical relevant project documentation. Relevant data may include NRCS near surface soil data , state groundwater data, well logs, topographic mapping, aerial imagery, geologic mapping.  Initial site visit with invitation to Forsgren Associates, including (1) Review site access and site safety, (2) Note general conditions for field investigation planning, (3) Photograph and document general site conditions, (4) Paint/stake boring locations, (5) Note potential overhead or buried utilities interferences that may affect exploration locations, and (6) Conduct property owner coordination.  Prepare project-specific instructions for Shannon & Wilson field personnel. Review the plan with Shannon & Wilson field staff and provide a courtesy copy of the plan to the project team. The plan will include preparation of: (1) Emergency medical (2) Safety requirements, (3) Vicinity map, (4) Tentative explorations location plan, (5) Schedule of field activities, (6) Required observations, (7) Logging instructions, (8) Testing locations and test intervals, (9) Test procedures and testing details, (10) Photographing requirements, (11) Subcontractor contact information, and (12) Utilities clearances matters.  Complete the following for drilling subcontractor, including (1) Prepare scopes of service for subcontractor, (2) Request quotation, (3) Prepare sub-agreement, (4) Collect insurance certificate, and (5) Coordinate contractor schedule of activities.  Prepare Form 1500 for cultural clearance and submit to LTHAC. Coordinate Form 1502 approval.  Upon receipt of ITD signed Form 1502, prepare Form 0649 for NEPA approval and submit to LTHAC. As part of the submittal, evaluate species and habitat. Attache approved Form 1502 to Form 0649.  Coordinate property access and field activities with Forsgren and the City of Rexburg.  Coordinate with DIGLINE to clear excavation locations.  Prepare a site specific health and safety plan (HASP).  Prepare to mobilize to the field (schedule equipment, perform equipment data collection calibrations, acquire supplies, etc.) and provide a kick-off briefing for field engineers and technicians.  Clear trees and brush to access drill hole locations. Mr. Brent Schiller, PE Forsgren Associates July 24, 2026 Page 4 of 8 Project No. 71-116716 Subsurface Explorations The “reliability and quality of the design will be directly related to the subsurface information obtained.” Subsurface data reduces the risks of “overly-conservative designs or costly failures in terms of construction delays, construction extras, shortened design life, increased maintenance, and public inconvenience” (ITD Materials Manual).  During the day for bridge abutment foundations, advance 2 borings to ~80 ft bgs at bridge abutments using HSA. As part of the drilling operation, perform SPT soundings, prepare field logs, and package samples for transport. Core 15 feet into bedrock, if bedrock is encountered. Backfill the drill holes with a mixture of cuttings and bentonite. Crew travel is included in this activity.  Scanline survey existing stream bed sizes (at proposed bridge and at 100 feet upstream).  Obtain 2 each riverbed soil samples for laboratory scour analysis gradations. Combine laboratory gradation with field scan data for combined particle-size analysis. Surveyor Coordination:  After completing the drilling, coordinate with FA surveyor to locate the structure borings (Elevation, Datum, Latitude and Longitude). Laboratory Testing  Unload samples, complete custody log-in, and store.  Review samples and prepare/coordinate testing instructions. Review test reports.  Obtain and organize photographs of rock core boxes.  At project closure, dispose of samples at a public landfill. This discussion and coordination meeting will be scheduled after the field and laboratory data have been collected and developed, and early geotechnical opinions have been formed  Perform general project management.  Remain available to answer questions. Note: Timekeeping descriptions will conform with the general work breakdown structure of Administration, Reconnaissance, Subsurface Explorations, Analyses, Reporting, or Other (with special notation). Key Understandings Our proposed scope of services is based on the following assumptions:  Design of a pavement section is not required. The City’s standard pavement section for pathways will be utilized as requested by the City.  Work hours are allowed from 7:00 am to 8:00 pm every day.  Site is accessible to truck-mounted drill equipment.  For better efficiency and productivity on out-of-town assignments, geotechnical- and materials-related field activities, such as drilling and testing, may involve 10- to 12-hour workdays. Our estimate is based on 10-hour workdays for fieldwork. Invoicing will reflect actual field time, including daily travel to the work site.  This scope of service specifically excludes the assessment of environmental characteristics, particularly those involving hazardous substances at the site. In the event that suspicious subsurface materials are encountered visually or by odor in the geotechnical test borings, such borings will be immediately terminated until Shannon & Wilson receives direction from the project Owner.  Traffic controls are not required.  Air monitoring or health and safety equipment beyond Level D is not required.  A USACE 404 permit is not required for the drill access plan. Drill holes will be situated above the ordinary high water line.  This is not a tribal area. Assessment of TERO tax is not anticipated.  Nighttime work is not specified for the materials investigation.  Shannon & Wilson may change subcontractors to accommodate schedule and other project goals. Mr. Brent Schiller, PE Forsgren Associates July 24, 2026 Page 7 of 8 Project No. 71-116716  GRS-IBS abutments are not planned.  Dewatering recommendations and services are not anticipated for this project. If the project changes and these services are requested, a separate scope can be prepared.  The materials scope of services presented herein does not include any structural design, scour evaluation, erosion control, project surveying, PS&E document preparation, or construction observation.  Subsurface studies are discovery investigations of unknown conditions. Laboratory tests, field tests and associated equipment and supplies estimated herein may not be used. On the other hand, some tests, equipment and supplies not initially envisioned may be appropriate to characterize the subsurface conditions and may be included as reimbursable costs. Schedule Considerations The following are suggestions for planning a schedule of deliverables. Activities Durations Reconnaissance/Subsurface Explorations Begin 15 days after kick-off meeting for 20 days Laboratory Testing/Analyses After Subsurface Explorations for 15 days Draft Geotechnical Engineering Report After Analyses for 15 days Sealed Geotechnical Engineering Report After receipt of client & owner comments for 7 days SUPPORT FROM FORSGREN ASSOCIATES This scope of service is prepared in a partnership with FA, who will provide information and support, including the following:  Notice to proceed  Right-of-entry onto the site  Location of ordinary high water lines.  Site plan and/or electronic topographic mapping  Prior studies for the site ABOUT THIS PROPOSAL In preparing this proposal, Shannon & Wilson used our experience to develop a scope of work, schedule, and budget for this type of project in this area. The level of detail of our Mr. Brent Schiller, PE Forsgren Associates July 24, 2026 Page 8 of 8 Project No. 71-116716 scope of work has an accompanying level of risk of unforeseen subsurface conditions and/or possible unanticipated circumstances associated with the project design, construction, and/or performance. Although risk can never be eliminated, more detailed and extensive explorations, analyses, and studies will provide more information to allow us and other designers to potentially reduce project risk and better meet project performance expectations. Because more comprehensive explorations and more detailed analysis and studies involve greater expense, we recommend client discuss the explorations and services offered in this proposal with us so that the services are sufficient for the client or owner’s budget and risk tolerance. CLOSURE We look forward to partnering with Forsgren Associates and the successful fruition of this project. Sincerely, SHANNON & WILSON Ryan G. Van Leuven, PE Geotechnical Engineer Charles E.
