To whom it may concern,
Attached is the UL investigation report for the sign at Grease Monkey. 2017 NEC requires all signs to be listed by an accredited laboratory (600.3, 110.3C). Based on the information
we have received, we cannot accept/approve this sign in the City of Rexburg. The signs need to be taken down and replaced or a field evaluation needs to be preformed by an accredited
laboratory. Either of these options need to be accomplished by June 1, 2022. We appreciate your cooperation on this issue. Feel free to contact me with any concerns.
Sincerely,

Hi Alex,
Confirming our phone conversation today, UL has determined the two UL labels reported in your e-mail below were applied to electric signs in the field without a UL representative present.
In addition, these labels that were applied to the signs are not legitimate UL labels. This is an unauthorized use of the UL Mark and not a permissible use of the UL Labels.
This case has now been referred to our Global Security and Brand Protection team, which deals strictly with matters that impact UL intellectual property rights.
Based on the above, we are closing UL Product Incident Report 2022MS-563. Thank you for your reporting this issue to UL.
If you have any questions regarding this report, do not hesitate to contact me.
Sincerely,

Lee,
I am the electrical inspector for the city of Rexburg, Idaho. I recently filed a product incident report, number 2022MS-563, because I went to inspect two signs on 585 N 2E in Rexburg,
Idaho and noticed the signs were not listed. When I informed the installer at IF Signs, he said he would get it fixed. When I went back there were stickers on the outside of the sign.
I asked him how they got there and he said he just field applied them. He told me Matrix LED Signs mailed them to him and told him to put them on for them. I checked the UL number on
the sticker and it is for a driver, not a sign. Please see the attached images (I am emailing you because the images would not go through on the incident report). Feel free to contact
me with any questions.
Thanks,
Alex Owens
