We believe that your company has the skills, exper�se, and experience necessary to complete this project successfully, and on �me. This project involves the replacement of five (5) dampers and four (4) damper motors with associated hardware. Based on our understanding of the project, we believe that it will require five to seven working days to complete. We are looking for a vendor who can provide the following deliverables: 1.Electrical Room Exhaust Damper 36”x48” with damper motor. 2.Electrical Room Supply Damper 36”x48” with damper motor. 3.AHU 200 Exhaust Damper 121”x61” rough opening: a.Two sec�ons 60”x42” each, with one damper motor. b.One building exhaust sec�on 36”x36” with one damper motor. 4.AHU 300 intake: a.1-each EBTron Thermal dispersion airflow measurement GTA116 Transmiter or equivalent. Remove and replace 36”x48” exhaust damper. Replace flashing around damper. Install new damper motor and integrate with exis�ng controls. 2. Disconnect the intake air handler from the intake damper. Remove and replace 36”x48” intake damper. Reconnect intake air handler. Replace flashing around damper. Install new damper motor and integrate with exis�ng controls. 3. Dispose of old dampers and debris. AHU 200: 1. Remove and replace 2 each 60”x48” exhaust damper. Install new damper motor and integrate with exis�ng controls. 2. Remove and replace 36”x36” Building exhaust damper. Install new damper motor and integrate with exis�ng controls. 3. Dispose of old dampers and debris. AHU 300: 1. Remove and replace the EBTron Thermal dispersion airflow measurement GTA116 Transmiter on AHU 300 air intake. Use EBTron or equal. Permits, Inspec�ons and Insurance 1. Provide all required permits and inspec�ons as required by the State of Idaho and the City of Rexburg. 2. Authority Having Jurisdic�on (AHJ) is the City of Rexburg Building Department. 3. Acord addi�onally insured cer�ficate and endorsement required prior to signing contract (see sec�on 13 of the enclosed contract template) Submission Guidelines 1. Proposal Format: Bidders are requested to submit their proposals in electronic format (PDF or Word document). Please ensure that all documents are clearly labeled and organized for ease of review. 2. Required Documents: Bidders must include the following documents in their submission: Lewis Corpora�on Page 3_______________________ • Technical Proposal: A detailed descrip�on of the proposed solu�on (Shop Drawings), including approaches to achieving the project objec�ves. Please address each requirement outlined in the Project Descrip�on sec�on. • Pricing Proposal: A comprehensive breakdown of all costs associated with the project, including labor, materials, equipment, and any addi�onal expenses. Clearly indicate any assump�ons made in the pricing. • Team Composi�on: Provide a list of key personnel who will be involved in the project, along with their qualifica�ons and Temple Recommend status. • Cer�fica�ons: Be prepared to provide upon request copies of relevant cer�fica�ons or licenses that may be required to perform the services outlined in the project. 3. Deadline for Submissions: All proposals must be submited no later than 6/22/2023 by 5 pm mountain daylight �me. Late submissions will not be accepted. Bidders are encouraged to submit their proposals well in advance to avoid any poten�al technical issues. 4. Submission Method: Proposals shall be submited electronically via email to Briansheetz@ChurchofJesusChrist.org. Please use the subject line: "Bid Submission – Lower-Level Damper Replacement Project - [Your Company Name]". 5. Site visits or mee�ngs may be scheduled during this phase to further discuss the project and address any specific requirements. No later than 7/10/2023. 3.Workdays are Monday through Saturday. Please clearly mark any proprietary or confiden�al informa�on in your proposal. 5.Disqualifica�on: Bidders not adhering to the specified submission guidelines, missing the deadline, or failing to provide the required informa�on may be disqualified from further considera�on.
