Observations/Remarks:
I arrived on site and began observations on the reinforcement for the footings from the southeast corner to the
northeast corner and to 300 feet west of the northeast corner of the main building. I looked for overlap,
clearance, correctness, and overall uniformity. I also made sure the reinforcement was lifted off the ground.
I observed that the reinforcement was within general conformance to project guidelines. I observed the
contractor manually tie the reinforcement and place it on the ground.
When I arrived I noted that the area being poured had been covered with concrete blankets the night before. At
11:10 AM the first truck arrived and I took a sample after approximately one yard had been expelled from the

I performed a concrete field test and observed that the results of the test were within general conformance to
project guidelines. I proceeded to take another sample from the fifth truck after approximately one yard had
been expelled from the truck. With this sample I performed a concrete field test. I observed the results of the
test within general conformance to project guidelines.
With each sample I made concrete test specimens and placed them in a cooler with heated water near the
footings to be covered with blankets and heated, I remained on site for the duration of the pour and reported my
results to the contractor then left the site.

Observations/Remarks:
Arrived on site and began observations on the reinforcement for half of the northern side of the footings and the
entirety of the western side. I looked for uniformity, overlap, clearance, and overall correctness. I observed that
the reinforcement was within general conformance to project guidelines, and that it was lifted up and off the
ground.
I observed that the contractor covered and heated the ground preparatory to concrete poured today. The truck
arrived at 11:55 AM and I took a sample at 12:00 PM. with the sample I performed a concrete field test and
made concrete test specimens.
I observed the results of the tests were within general conformance to project guidelines, and I reported my
results to the contractor.
I left the concrete test specimens in a cooler with heated water next to the footings to be covered and heated
overnight then left the jobsite.

Observations/Remarks:
I arrived on site and began observations on the footings north and west for the main building.
When I arrived the third truck was in the middle of pouring and almost finished. I waited to take a test from the
fourth truck. At 12:25 PM I received a sample after approximately 1 yard had been expelled from the fourth
truck.
With the sample I performed a concrete field test and made concrete test specimens. I observed that the results
of the test were within general conformance to project guidelines.
I reported my results to the contractor and left the job site. Prior to pouring I noted that the contractor had
covered the ground to be poured and had heated it overnight.

specimens. I noted that the air was a little high and informed the contractor who proceeded with the pour I
observed that the results of the tests were within general conformance to project guidelines. I reported my
results to the contractor and left the jobsite.

Observations/Remarks:
I arrived at the job site and met with the foreman to discuss the pour. We also talked about a good place to
setup for testing.
I prepped my gear and waited for the first truck to arrive. The first truck arrived around 10:45, I grabbed my
ticket and waited the allotted time to grab my sample. I did so and returned to my work area to begin my
testing.
My results were within spec and I reported to both the driver and the foreman.
I returned to my vehicle to clean my gear. I observed the remainder of the pour and began my report.
I left the job site around 12:45.

Observations/Remarks:
I arrived at the job site around 11:30 am and began a rebar inspection. I found the rebar to be up to spec. I
performed other tasks while waiting for the first truck.
The first truck arrived around 12:00 pm and I was able to grab a ticket from the driver and then waited for her
to have a yard off. Once a yard was discharged I was able to grab a sample and begin testing, the results were
within spec and reported to both the driver and the foreman. I poured my cylinders and then cleaned my gear in
preparation for my second test.
The fifth truck arrived around 1:15 pm and I was able to grab a sample and begin testing around 1:30 pm.
The results were also within spec. I placed my cylinders and began cleaning my gear. I observed the remainder
of the pour.

The fifth truck arrived around 1:45 pm and I was able to grab that ticket as well as the sample once the allotted
time had passed. After running my tests I found the results to be within spec once again. I was able to pour my
cylinders and place my cooler.
Once I finished my work, I began cleaning my gear and returned to my vehicle. I began filling out my report. I
left the job site at 3:00 pm.

Observations/Remarks:
I arrived on site around 9:30 am and was able to check in with Bateman-Hall to see where the best place to set
up would be. I parked and set up my testing equipment.
As we were given late notice, I was only able to obtain a sample from the third truck instead of the first. I tested
for air, slump, and temp. All were within the given specs. After testing I made strength cylinders.
I was also able to test the sixth truck in line and the results were acceptable as well. Results were passed on to
Bateman-Hall as well as Walters Ready Mix. While observing the remainder of the pour, I noticed that mechanical
vibration was being utilized throughout the pour.

Observations/Remarks:
I arrived on site around 10:30 am with enough time to look over the forms and rebar. Spacing and size appeared
to be correct.
Concrete showed up right on time at 11:00 am and after displacing a yard, I was able to obtain a sample for
testing. I tested for air, slump, and temp. All were within spec. I reported that back to Walters and Bateman-
Hall.
After making strength cylinders I cleaned up my gear and observed the duration of the pour. I did notice that
mechanical vibration was used on the walls.

Observations/Remarks:
I arrived at the job site and looked over the rebar as well as the forms. The spacing was correct and so was the
ground clearance.
Once my inspection was completed, I returned to my vehicle to setup my equipment. I then waited for the truck
to show up.
The truck arrived around 11:15 AM and I was able to grab the batch ticket. After waiting for the driver to offload
about a yard and a half, I was able to grab a sample and begin testing.
I tested both air and slump, both of which were within spec. I reported to both the driver and the foreman.
I then casted strength specimens. Once completed, I placed them and began my cleanup.

place to set up would be that was out of the way.
After setting up my gear I went over to the wall forms to look over the rebar. I checked for correct size, spacing,
and to make sure no rebar was touching the forms. All appeared to be in good order.
Once the concrete arrived I was able to obtain a sample for testing. I tested for air, temp, and slump. All results
were within spec and reported to Walters and Bryan Landon Construction.
After testing, I made strength cylinders and then cleaned up my gear. Upon completion I was able to observe the
remainder of the pour. I did notice that mechanical vibration was being utilized to consolidate the concrete.

Observations/Remarks:
I arrived on site at 4:30 AM, contractor was on site setting up their equipment. I walked around checking on the
forms to make sure the rebar was within spec. The first truck arrived around 4:50-5:00 AM. We grabbed our first
sample around 5:10 AM. We tested every 50-60 yards of the pour, and did a total of 11 tests.
We finished testing and started to clean up around 10:15-10:20 am.

Observations/Remarks:
I arrived on site around 4:30 am. I got out of my vehicle to go inspect the rebar placements and found it to be
within general conformance to project guidelines. I then proceeded to set up my testing equipment for the day.
The first truck arrived around 5 am. I got my first sample around 5:10-5:15 AM. All tests resulted to be within
spec. And were reported. I did a total of 7 tests.
I observed the duration of the pour and wrote this report.

Observations/Remarks:
I arrived at the job site around 9:15 and looked over the rebar. It looked good for the most part, I spoke with
the crew about some question areas. And the crew solved the issues.
I returned to my vehicle to setup my equipment.
The first truck arrived around 10:20, I was able to grab a ticket and wait to grab a sample. Once I received a
sample, I returned to my work area to begin my testing. I tested both air and slump, both of which were within
spec.
I reported the results to the driver.
I cleaned my equipment and left the job site.

Observations/Remarks:
I arrived on site at approximately 4:55 AM and proceeded to immediately set up my testing equipment in
preparation for testing. Shortly after I inspected the rebar placement inside of the forms and found them to be
within acceptable specifications.
When the first truck arrived on site at approximately 5:00 AM I proceeded to acquire a sample and test. The
tests were acceptable and passed within the given parameters. However when testing the truck for 50-60 yards I
found the air quality to be lower than passing. By the time the test was finished, the concrete had all been
poured and the truck had left. I then tested the truck with 80-90 yards and found all tests to be acceptable and
within the given parameters.

I then cleaned my testing equipment and observed that the contractor used a mechanical vibration device to
properly displace the material. I observed that an employee of the contractor stayed behind after other
employees had moved on to ensure that the concrete was all finished correctly.

Observations/Remarks:
I arrived on site around 4:45 AM. Shortly after arriving and parking, I exited my vehicle and began to inspect the
rebar placement, and found it to be within general conformance to project guidelines. Afterwards I began to
setup my testing equipment and wait for the first trucks arrival.
The first truck arrived at 5:00 AM. I grabbed my sample between 5:10-5:15 AM. I concluded all of my tests with
satisfactory results and informed the contractor of the results.
I observed the contractor use all of the correct procedures to make sure the concrete had the right displacement,
as well as the correct level and finish. They used mechanical vibration for the correct displacement of the
material.

I arrived on site at approximately 4:55 AM and proceeded to immediately set up my testing equipment in
preparation for testing.
As soon as my gear was prepared, I acquired a sample from the first truck at approximately 5:20 AM. When
testing this sample the air quality was .5% lower than passing. The truck had already finished pouring by the
time the test was completed. I then tested the third truck at approximately 5:55 AM and found all tests to be
passing within the given parameters. When the sixth truck arrived I tested it and found it also to be passing

I then cleaned all of my equipment and gear. I noted that the workers made sure that the concrete was set

I arrived at the job site around 7:45 AM. I looked over the forms and spoke with the foreman about the mix and
the supplier.
I returned to my vehicle and began setting up my equipment. The truck showed up just after 8:00 AM and I was
able to grab the batch ticket. Mike with Knife River showed up shortly after.
I observed as they offloaded some of the mix and was able to grab a sample after they offloaded about a yard.
I returned to my vehicle to begin testing air and slump. Mike observed as I tested and recorded results as I got

The first truck arrived around 4:40 AM. I sampled and performed a concrete field test. I reported my results to
the contractor. I then took a sample every 50 yards performing a field test with each sample. Strength
specimens were made for each sample that was taken.
