THE CONTRACTOR IS REQUIRED TO MAINTAIN A COMPLETE COPY ON SITE AT ALL TIMES(RESOLUTION 2016-15).2.ALL DISTANCES AND DATA SHALL BE CHECKED BY THE CONTRACTOR PRIOR TO THE START OF CONSTRUCTION. IN CASE OF CONFLICTTHE ENGINEER SHALL BE NOTIFIED IMMEDIATELY SO THAT CLARIFICATION MAY BE MADE PRIOR TO THE START OF WORK.3.BASIS OF BEARINGS CITY OF REXBURG CONTROL.4.ELEVATIONS SHOWN ARE NAVD 88 DATUM TO FINISHED SURFACE OR PIPE INVERT UNLESS OTHERWISE NOTED.5.GENERAL LOCATIONS AND SIZES OF UTILITIES ARE SHOWN ON THE PLANS. THEY ARE TO BE USED FOR GENERAL INFORMATION ONLY. ITIS THE RESPONSIBILITY OF THE CONTRACTOR TO NOTIFY THE APPROPRIATE UTILITY COMPANIES WHEN CONSTRUCTION MIGHTINTERFERE WITH NORMAL OPERATIONS OF ANY UTILITIES. IT SHALL ALSO BE THE CONTRACTORS RESPONSIBILITY TO HAVE THEAPPROPRIATE UTILITY COMPANY FIELD-LOCATE ANY UTILITY INSTALLATIONS WHICH MIGHT BE AFFECTED BY CONSTRUCTION PRIOR TOBEGINNING WORK IN THAT AREA. THE CONTRACTOR SHALL BE RESPONSIBLE FOR MAINTAINING SERVICE OF EXISTING UTILITIES ANDFOR RESTORING ANY UTILITIES DAMAGED DUE TO CONSTRUCTION AT NO ADDITIONAL COST TO THE OWNER.6.THE CONTRACTOR SHALL BE RESPONSIBLE FOR SECURING SOURCES FOR GRANULAR MATERIAL, DISPOSAL SITES, AND ANY ADDITIONALMATERIAL THAT MAY BE NECESSARY FOR PROPER CONSTRUCTION OF THIS PROJECT.7.IT SHALL BE THE RESPONSIBILITY OF THE CONTRACTOR TO MAKE ARRANGEMENTS FOR WATER REQUIRED FOR TESTING, COMPACTIONAND DUST CONTROL MEASURES.8.ROCK EXCAVATION MAY BE ENCOUNTERED DURING UTILITY CONSTRUCTION. EVERY EFFORT WAS MADE TO REDUCE UTILITY DEPTHWHILE STILL MEETING REGULATORY REQUIREMENTS. FOR BIDDING PURPOSES CONTACT OWNER FOR TEST HOLE DEPTH OF ROCK.9.DEMOLITION IS REQUIRED ON THIS PROJECT PRIOR TO JOB START. 1. CONTRACTOR IS RESPONSIBLE FOR THE OFFICIAL WRITTEN SWPPP AND NOI MEETING EPA REGULATIONS.
2.CONTRACTOR SHALL INSTALL A DIRT BERM OR SILT FENCE AROUND THE PERIMETER OF THE SITE(S) WHERE EARTHWORK IS TAKING
PLACE TO LIMIT POLLUTANT DISCHARGE LEAVING THIS SITE AND ENTERING ADJACENT PROPERTIES. THIS CAN BE INDIVIDUAL
BUILDING SITES OR ENTIRE PERIMETER. CONTRACTOR TO MAINTAIN BERM OR SILT FENCE REGULARLY TO ENSURE SEDIMENT AND
OTHER CONTAMINANTS FROM MIGRATING OFF PROJECT SITE.
3. CONTRACTOR TO ESTABLISH A STAGING AREA FOR FUELING AND ENSURE COMPLIANCE WITH SPILL PREVENTION AND CONTAINMENT
BEFORE THE START OF ON SITE CONSTRUCTION ACTIVITIES.
4.CONTRACTOR SHALL ENSURE TRUCKS EXITING THE PROJECT SITE ARE NOT TRACKING MATERIAL OFF SITE. IN THE CASE TRACKING
MATERIAL OFF SITE OCCURS, THE CONTRACTOR SHALL HAVE A PICK-UP BROOM (OR OTHER EQUIPMENT) ON SITE TO ENSURE THE
OFFSITE ROADWAYS REMAIN CLEAN. THIS WILL BE SCHEDULED ON AN AS NEEDED BASIS.
5.IF THE CONTRACTOR DOESN'T CONTROL THE TRACKING OF MATERIAL ONTO THE CITY/COUNTY ROADWAYS TO THE SATISFACTION OF
THE CITY, THE CITY MAY SHUT DOWN THE PROJECT UNTIL THE CONTRACTOR INSTALLS ANTI TRACKING PADS (STONE OR
CORRUGATED STEEL PANELS) AT A DESIGNATED EXIT POINT PRIOR TO EXITING THE SITE.
6. CONTRACTOR TO INSPECT EQUIPMENT/VEHICLE STORAGE AREA AND FUEL TANK WEEKLY AND AFTER STORM EVENTS. LEAKS TO BE
REPAIRED IMMEDIATELY. SPILL CLEANUP MATERIALS SHALL BE AVAILABLE ON SITE AND PROPER DISPOSAL OF CONTAMINATED
MATERIAL SHALL BE DISPOSED OF PER EPA GUIDELINES.
7. INSTALL CONSTRUCTION RACK/MAT, SHAKER PLATE, RUMBLE RACK OR EQUAL TO MITIGATE TRACKING OFF SITE. CONTRACTOR TO
SUBMIT SPECIFICATIONS TO ENGINEER FOR APPROVAL.
