Observations/Remarks:
I arrived at 1:20 pm, and inspected the rebar and forms. I was able to get my work station setup while the pump
truck was setting up.
The trucks began to arrive around 2:00 pm and I was able to get my sample and begin my testing. The results
were received and reported, the air was low but they decided to continue.
I cleaned up and observed the remainder of the pour. The crew utilized knocking on the forms for consolidation
as they went. I returned to my truck and finished my report.
I left the job site at 3:00 pm.

Observations/Remarks:
I arrived on site and prepared my gear. I noted a pump truck was to be used to place the concrete. At
approximately 5:30 am, the first truck arrived. After approximately two cubic yards had been poured, I sampled
from the truck and began my tests. I found these tests to be within the given specifications. I then sampled from
the fifth truck, and performed the required tests. I found the tests to be within the given specifications, except
for the slump test. The specification for slump was 6 in and I tested 6 3/4 in. I informed the contractor of my
results. After performing my tests, I observed the remainder of the pour. I then returned to the lab.

Observations/Remarks:
The rebar was inspected, and photos placed into this report.
A Walter's Concrete truck arrived a half hour later than scheduled. The concrete was placed via pump truck
(Brundage Bone). After approximately 10% of the truckload was offloaded, a sample was collected and tested for
temperature, unit weight, air, and slump. Compressive strength cylinder specimens were casted and stored in a
thermal container for initial set.
Results were reported to K&C Concrete, as well as Walter's mixer driver.
Two trucks total (16 Yds) were placed. The entire pour consisted of the continuous footing around the perimeter
of the building.
