Attached is the contract to provide service to your new charging stations. Please sign and mail it with your advance of $7,660.44 if choosing the $250 non-refundable option along with
a signed ESRA document that is attached, to the Rexburg office. Your request for service will be scheduled and prioritized based upon when we receive those items.
Please carefully review the enclosed contract and note that the contract will expire unless you:
a. Sign and return the contract along with any required payment or other information to the Company within 90 days of the contract date shown on page 1 of the contract, and
b. Are ready to receive service within 150 days of the contract date.
c. You are required to forward this e-mail to Byron Borg who will be your contact for inspection and scheduling. You are also responsible to initiate contact with Byron Borg one
week after we have received the above items. Byron will inspect, schedule and manage the construction of your installation and can be reached at Byron.Borg@pacificorp.com <mailto:Byron.Borg@pacifico

d. Rocky Mountain Power is currently experiencing supply chain issues and our ability to procure materials needed to complete your construction project may be impacted. We will
continue to design projects as requested and materials will be ordered upon receipt of signed contracts and money. We have limited control over the delivery/lead-time of the material
and will schedule your project as soon as the needed materials are delivered. We apologize for the inconvenience. Thank you!
