HomeMy WebLinkAboutSPECIAL INSPECTION FOUNDATION - 21-00208 - Goldenwest Credit Union - Branch Office1594 Park Circle Ogden, Utah 84404 801.782.6008, Fax 782.4656 arwengineers.com Construction Observation Report # 1 Project GWCU – Rexburg Branch Project Number 19473 Location HWY 191 and University Blvd Date & Time Tuesday August 10th, 12:00 p.m. Architect BPA Weather Sunny, clear Contractor R&O Observer McKay Parrish Persons Present: Steve Welch – R&O, Cade – Virgin Concrete Project Status Summary: Foundation walls are being formed Items of Note: I met with Steve on site and we discussed the following items: Item No.: Item: Follow-Up / Action Required: Initials Photo: 1-1 Rebar and anchor bolt shop drawings were not submitted and no rebar cages were installed for column piers or at holdown locations as shown in details 3/S2.2, 4/S2.2 and 1/S2.3. It also appears that all vertical wall dowels were wet stabbed into the footings. Because the footing concrete is not fully cured (it was poured on Monday, August 9th), dowels using epoxy into uncured concrete have a significant reduction in tension capacity. Several options to address the missing reinforcement were discussed, including the option to lengthen and lap the threaded rod anchors at holdown locations. The contractor submitted RFI #3 for ARW Engineers response regarding repair options. #1, #2 1-2 All anchor bolts are to be tied in place prior to pouring the piers and foundation walls. The anchor bolt types and locations were discussed with the contractor. The contractor indicated that they would tie anchor bolts in place prior to pouring piers and walls. Questions regarding column locations were discussed and the contractor was advised to coordinate all dimensions with architectural drawings and to submit anchor bolt shop drawings for review. N/A 1-3 As indicated in note D.1.d on sheet S0.1 and note 8 in the footing and foundation notes on sheet S1.1, the contractor is responsible to verify footing elevations and grades prior to pouring the footings and walls. All exterior foundations shall bear a minimum of 36” below the lowest adjacent final grade as indicated in the structural notes on sheet S0.1 and in the Geotechnical Report. Cade (Virgin Concrete) indicated that he was aware that the bottom of footings did not appear to be low enough prior to pouring the footings. At locations where finish grade to bottom of footing is less than 36”, the contractor is to pour a 6” thick concrete frost wall around specific locations of the building as shown in the attached detail. This skirt is not required along gridline K where the footings step down for the drive-up canopy, or at locations where concrete pads or walks are set at finish floor elevations. This frost wall can be soil formed, 3,000 psi concrete, and does not require dowels or reinforcement. Detail 1 1-4 The cantilevered column footing at the drive-thru canopy was discussed, and because anchor bolt shop drawings have not been submitted, the proposed anchor bolts on side were not correct. The contractor indicated that they would install the 1” diameter A449 anchor bolts at these columns as indicated in detail 2/S2.2 N/A Construction Observation Report GWCU- Rexburg Branch Page 2 of 4 Items of Note: I met with Steve on site and we discussed the following items: Item No.: Item: Follow-Up / Action Required: Initials Photo: 1-5 Rebar clearance around foundation wall penetrations was discussed. There are foundation wall blockouts proposed in various locations around the building. At locations where blockouts occur, the contractor is to adjust the rebar as required to allow for required rebar clear cover as indicated in detail 4/S2.1 and as outlined the structural notes. N/A 1-6 Dowels between the foundation wall and interior slab on grade were not yet installed. As shown in detail 7/S2.2 and 8/S2.2, the contractor is to install #4 dowels at 24” o.c. around the entire structure. At the time of our visit the walls had not yet been formed and dowels had not yet been installed.
