Enclosed is a quick reference map sha��ing the approximate locations of the manholes and storm drain rates with a reference number hiAl'oh-ted in blue with comments 11sted below and each of the problems we found. Site 4 1. Broken grade ring - Site #2. Racks used to shim to grade Site ob Concrete circles, unknown what's under them. Site #4. Broken concrete under grade, Slate #5. Full of debris. Site 4,6. Fuld of debris. Site #7. Full of debris. Site #S. Nuts used to shim to grade, not .grouted. Site #9. Broken and cracked grout. Site # 10. Broken grade ring, broken barred _ Site 411.
