On Wednesday, April 1 4' a construction site visit was made io the above referenced project.. Based
upon observations made during that visit, this office requests a written response to the following
issueslitems noted:
} In my January 1 9t" letter to your office I mentioned measurements taken on the wl'dffi
of doors #1 07 to the exterior from End #170. In that letter I stated each door leaf
only measured forty-two (42) anehes wide and that the center mullion between the

2} In that same letter, I again commented on the openings in the vertical separation wall
between the existing lower level central yore and second floor Com'dor #245. As
mentioned in previous correspondence, applicable hospital ficensm'g regulations
require a runimum one (1) hour vertical separation which would not be provided by
openings vviti no glass, plain glass, or even weed glapSi, if not restricted in size. In
a packed of information faxed to your office an January 151h, 1 provided you wth
copies of an acceptable aiternaie (i.e., equiwalencyT} appoach thai could possibly
be used to determine what could be done at these openings to meet equivalency.
During my March visit to the facility I Holed NNired glass installed in the openings,
but of a size gzeater khan allowable by code. This office would appreciate
information justifying the use of individual glass panes exceeding 1,296 square
inches. If an equivalency approach was applied/met, please provide this office with
the supporting documentation for our review/verification,
3 } In my June 18, 1998 letter of review an the drawings and project manual for the
referenced project, I commented in. Item B)1) that at a minimum one (1) smoky
barrier paftition wall would be required on the second floor and that none had been,
shownf indicated. I wend onto made a suggestion as to where the wall couldJshould
run and commented on the number of HVAC duct penetrations. In a July 14, 1998
letter from your office, Item B)1) was addressed by the statement, "Refer to
drawings G-3.1, since the hospital has ap#eco to construct the T.Q.U. shell only,
the upper level has been di"'ded up into (3) smoke compartments, the T.C.U.
shed, the nursery/delivery area and the LDRP wing. The Mechani'cal Engineer
shall be consulted to insure proper smoke daggers, etc.. The wail that you have
suggested 'is in fact one of the smoke barriers". In monitoring the wall between
the LDRP wing and the nursery delivery area, I question whether the requirements
for a smoke barrier partition wall are being addressed. There are numerous
penetrations in the wall up to the roof deck and around beams, ducting, etc., the
corresponding wall above the crass corridor doors may/may not be berg addressed
i.e., no wall hype indicated on G-3 . 1), and there Ys no indication the HVAC duct
penetrations havp. Allbeen addressed. requirements for bus wait need to be reviewed
to be certain appliabie requirements are mei and that the cantracta is awake of the
requirement.

4) And nall)T, this office requests a written staterrent, whether ft be from your office
as the hospital's representative, or from the hospital's adrninistratian specifically
regarding anyla.I, "shelled" spaces that may remain upon project comp ion. As I
staffed in my review caxnnents on drawings(i.e.,, specifically sprinkler shop
drawings), any/all shelled space(s) Will require sprinkler coverage which has got
been shown/indicated,
Should you nave any questions, please coil or write.
