STRUCTURAL STEEL WELDING INSPECTION REPORT
On the above date, our representative performed work an the referenced project as reported below.
The fallowing conditions were observed during a general site observation.

Ledgef Angie etc, was not properly cleaned upon completion. The welding was unacceptable due to improper
p o s t welding eleanL'Tag in accordance with Paxagraph 5.30.2 "Clearung aF Completed Welds. Slag shall be
remov�d from all completed welds, and the weld and adjacent lase metal shall be cleaned by brushing or ether
suitable means" AWS D 1, 1-2002 Structural ANVelding Code — Steel.
2: It was observed that there were various locations that the fillet welding of the ledger angle to the embed
plates, rube steel tc) joist, and fabricated beam stiffner plate welding was incompletc.
3-- It was observed that thore were various locations that the fillet welding of the tube steel to the joists vas
unacceptable due to undercut in accordance with `able 6.1 of ASS_ DI. 1-2002 Structural Welding Code Steel.
4: It was oloserti•ed that the beam detailedon Sheet S101, Grid Line F.9 Between approximately 2.5 and 5.3
was spliced to achieve overall len�th. A copy of RFJ #43 was reviewed and the beam. splice was not full-

the base metal of the beam was not prepared in accordance with Paragraph 5.X7.1 of AWS D 1. 1-2002 Structural
Welding Codi — Steel.
5: If was observed that here were temporary brackets utilized to aid in the structural steel erection of the
b,asement beams. The additional brackets and welding willrequirelil engineers approval in accordance with
paragmpla 6.5. E DofA�S1. 1-2002. Structural Weld'ng Cede --Steel. if approval is acquired, the welding shall
be,- subject to the same welding .procedure requirements as the final welds in accordance wi.tb Paragraph 5,18,.1
of AWS D1.1-2002 structural welding Cade — Steed.
6: It was observed that various Joists that were not vve�c�ed in accordance the project drawing due to
fit-ur) restraints. Approval of the changes in the required wedding as detad will require Engineers approval in

On the above date, our representative performed work on the referenced Project as reported below.,

2. Welding has been completed on Spliced beam in the basement area. Beam was tiot prepared ill
accordance with code requirements. UT is scheduled for noon o�riday April I I ��'.
3. The roof deck has been completed #'r4m Lines to 1 3. There were mechanical fasteners missing in

4. The base plate at the conier
of Line 107 Row 111,1 7 is unacceptable due to size of fiame cut hole-. Welder
says he has put as much veld on this anchor bolt to base plate as he can. Shawn Reeder, Architect, said
it would be okay with a 5/ 1 6" fillet weld. Weld is defici�eni.
