After rev�ew�n� the 199? Uniform Building Code and the Plan� fay the Benson.
building I have found that the stairs of the building are not within the limits set by the
building code of 7" Maximum rise and all three of the stars in question are not built as
drawn in the plans shown on pales A401 and A 402. The 3/8""s variation can only apply
to steps that have a maximum rise of 7" as prescribed by the code, .x,11 of these steps are
over the 7" limit. I have not seen the change artier that dropped the bottom landing 6"" so
I do not know if the architect specified any dimensions to the steps for the change. All
three of these steps ars not within the limits of the Cade and are not as drawn on the plans
therefore they should be corrected and brought in compliance with the cade and with the
plans.

Additionally, I received a call from that firm's Manacyer, Mr. Dan Kong ca1lino, my
attention to the existence of the faxed reports, and also indicatl'na that
understand that your contractorhe teras given to
, Jacobsen Construction, had caused the removal of the
Special Inspectors, and directed that they could not return to the s�tc.
To reiterate the position of The City of Rexburg in this mater, if there is no Special
Inspection as required under Chapter 17 (Structural Tests and Inspections) ffitemational
Bul"Idincy Cade 20d0, the Build'sng Dept. would hold your build'

technically 'in suspension, pending resalution �fthe matter of any on--siie Zack of

At your request I performed a welding inspection/observation on the already completed 5icybridge 'in the
new section of the Benson Building from grid line 2.0 to 3.0 and from G-.9 to the existing building. All phases
of the construction for this item could not be observed but approximately 90 °Io of the bolting and welding
could be seen. After reviewing the plans, the general contractors daily reports, and the completed work, it is
TQM Consulting opinion that all work per -formed is in compliance with plans, specifications, and applicable
building. codes. However this observation in no way creates liability for TZM Consulting, nor does it relieve
the Independent Testing Agency responsible for the project inspection and the contractor who per -Farmed
the work, of their liability for the Benson Building Skybridge being ars accepted structural idem.

The addition to the Benson Building has been completed and inspections by the
Building, Fire, and Plumb
irig departments have been completed satisfactorily. Only the
Ele - ctrical inspectors signature is needed to issue the permanent Certificate of Occupaney.
As per verbal. confirmation with the Qt3TCIerk there are a few minor items that need ta
be com.pleted. This letter is to give the ager, BYU xdaha, a Temporary Certificate of
Occupancy and to allow then to start moving in and occupying the building until the
electrical inspection is completed and the Permanent Certificate of Occupancycan be
xssued.

This is an amendment to our previous fetter in regards to the Benson Building Skybridge. At your request
TQM Consulting has performed a end inspection on the already completed Skybridge in the new section
of the Butson Building from grief dine 2.0 to 3.0 and from G_9 to the existing building. We utilized a torque
wrench and filet weld gauges to da a thorough investigation of the structural items pertaining to the
Skybridge. All welds, high strength bolts, s#rruetural steel, and epoxy anchor baits appear to meet plans and
specifications. Approximately 10°% of the items were not aC ble, which stil meets the UBC Section
1701,5 Item 5 for, periodic inspection of fillet welds and the AISC "5,pecificatians for Structura( Jo-Ints {sin
ASTM A325 or A490 Boits". it is T2M Consultingts professional opinion that all work performed is In g
compliances with all applicable building roles and project documertation. Tank you far your time. We look

1. Storni Water Run-off, The University has contracted with Keller and Associates to conduct the
master planning for the development of the remainder campus property to 7t'' south. The goals of
the study are to maintain order in any possible future -expansion and manage run-off. The plan
will manage the storm water on campus property for slow release into the City Storm Sewer
+
system. The new roof area for the addition will drain into existing piping and thus into the
existing 5tom1 Drain System. Plans for this work will bc; submitted to: the City Offices for
review.
2. Accessible Parking. The University currently provides 46 off-street accessible parking stalls,
The addition of 5 new stalls into the Benson parking lot will require 7 traditional stalls be
removed. ADAAG, 4.1.2 Accessible Sites and Exterior Facilities: New Construction, 4. I.2 (5)5
Table, requires that the number of stalls be calculated based on parking lot size. If the Entire
Campus is considered, 20 stalls plus I per 100 are required. We have a total of 2760 parking
stalls that requires 20 + 1760/100 * I — 3 8 stalls. There are 6 stalls in the TaylorlBenson Parking
lot of 153 stalls where 6 are required. There are an additional 2 stalls on Center Street. From
these calculations, it appears that there are adequate stalls on cainpus and in the iinmeciiai:e area.
Our -preference is not to add accessible stalls in this area.
If this is not an option, we would offer
to install 2 additional stalls adjacent to the existing cross walk opposite the existing accessible
stalls. (We are currentily preparing the Multi -Use Building parking tot for use. This lot contains

3. Fire lane access. I held a telephone conversation with the Fire Marshall regarding the fire lane.
The lane as shown on the site plan will be a temporary lane for fire and construction access. The
width is planned @ 16'. The lane will be constructed from IW" road base and gravel to support
concrete trucks and fire apparatus. The site be scarified 6" below existing grade.

Sorry for taking so long to get back with you. Typically we have an approximate
tine of tarn around on plan review of about 2 weeks from submittal. Recently that ti'me
interval has increased to a little. The last few days have been extremely busy for us.
Z have reviewed the plans for Ben -sen Building Addition at the BCU -Idaho campus
and have identified the following discrepancies:
1. According to
the
code analysis on sheet
F 10 1 a possible additional 5 9 1 students
could occupy
the
bufldm'g,
herefore an
additional 148 parking spaces would be
need to be provided. The University has plans to increase parking an an eniire
campus bevel. Therefore, an additional parking dot is not required in close
proxmty to the building. However, additional handicapped parking is still
required. ADA requirements identify the need for 5 additional handicapped
parking spaces.
2. Our policy on storm drainage requires that you submit us with a pian on how
you will handle the additional storm runoff creased by the addition. This plan
mush address the storm water detention so that the new flows into. the City storm
sewer are metered to meed those existing prior to construction. The plan
submitted does not meet our criteria.
3. Provide information an fire -rated door and window assemb�ies. I didn't see a
door schedule. We will need detailed �nfarmativn on all doors in the rated

corridor as well as those at the area separation wail. The existing door neap the
middle entrance between the two buildings will need to be gated due to its
proximity to the addition.
4. Provide a handicapped ramp at the North entry. As the design is now, only
stairs are existing at this entrance.
5. The handicapped ramp at the fast entry does not have 5' wide landings as
required by ANSI Al 17,1-1998, Section 445.7.4. The landings must be 5' wide
M each direction.
6. Z have attached a dist of discrepancies identffled by our new Mechanical
Inspector as well as a report created by the Fire Department.,

recognize the, need of the City by'be apprised of all change Orders, Field Directives, etc.
(without cost factors) in order that they may be reviewed to ensure that the intent of the
Codes, are recognized, or provided for.
MR -2: (General Conditions- Sec. 17-A 1.2&3-Inspecton of Work.) The Contractor of
Project Coordinator shall notify the City of work in all instances of Systems and piping in
progress and status so that the work may be viewed prior to backfill and incorporation
into the project. Additionally, the City shall be advised when special inspections are
scheduled and City inspectors are invited to view the inspection and receive all reports
generated there from.
A list of all firms engaged by the owners, A/E. or contractor shall be provided the City,
