Forsgren inspected the masonry wall between B 1 and half way to F 1. The upper 4 feet of
walk had. not been grouted. On JanuaryI 5t a blizzard brought winds up to S mph to the
Rexburg area., and scaffolding and tenting that surrounded the wall was damaged. An
�mp�vyee of Mickelson says $1 Q, 000 of scaffolding was bent and ruined when it toppled.
Forsgren observed that much of the upper section of wall from eight to twelve feet height wzXl have fo be removed due to extensive �rack�ng of mortar 'Dints a�ad d

Forsgren inspected the placement of masonry block and reinforcing steed at the north
extenar wall of Deseret Industries from A. I to A-4 from a height of twelve to eighteen
feet and from A-1 running southward towards $_ i to the first Boar, ftom a height of
twelve to sixteen feet.
Forsgren witnessed the grouting and vibrating of the masonry c.oluxnns and beams.
Temperature and slump were tested (G0 deg. F. and 9-1/2 inches, respectively). The six
CY batch was truck -mixed b� Walters a� its Vailey Concrete facility in Idaho Falls under
Invoice 0458Q. Aggregate of minus 318 inch screen and 7 -bag mix were used, aeon with
5 ounces per Cy of Polvheed. Ambient aar temperature was 29 deg. F. and ei htgallons
of v�a�er vvas added to t�h.e mix �� die site.�
No samples were taken.
Forsgren found the work inspected to be in compliance with the approvedplans andspecifications.
