example of is on 24 Sept
30 December 2003
. an inspection was performed for puddle welds and studs lines
L J.3toL 4.7 and C 3.3 to U4.7. 42 suds failed but no mention of weld failures. an 26
Sept. 60 cy. of concrete was poured in the same area with no mention of slump or air
tests. We have only received test results on concrete up to July 18 with no mention of
pass or fail. The compressive strength is listed but I have no wad to dell the strength
required, an 30 Judy SS cy_ of concrete was placed at dines C4.7 to J.5 t0 4.7 then again
on 31 July t was poured again. On 3 November a boli inspection was performed but no
location. On I I -I P1i a statement was made that all suds appeared to meet AWS D 1. 1.
Did thev pass or not? There are severalpaces dike this with inspections completed Z-3
times for the same area and some with no location mentioned. The nelson suds ars the
only things I can tell that do pass.,
We cannot go on the assumption that if the report does not say "fail" that it
basses. For compression tests we need the strength required and strength achieved. We
need test results, location of the tests and a pass/fail
I have several questions about submitted reports listed below.
fo RICK Are bolt and weld inspections being performed on beams/columns before
concrete is poured on an elevated slab `s I found many bolts missing along line M4 to M7
after the floor was poured. Some beams were only held by a couple of bolts and spit
welds.
2. RICKS Have any rebar inspections been performed. I save Clayton dot -ng some but
there is no mention of any being completed in any reports.
3. RICKS Why are there no bests for g -rout or lean mix concrete,,
4.. RICKS Noreport for any tests for the slab at K4 -ISS to,M4-M8
5. RICKS We have not received any reports since S December we need them for a timely
review to insure carrections are made, if any. A report a month after the fact makes it
very hard for the contractor to made corrections in many cases.
6. RICKS Why aren't there tests for gTout or lean mix concrete? isn't it basically a
footing for a footing?
7. RICKS There are at least 8 pours with no Nests done? 6-25,, 10-3:> 1 10-23, 111-5r 11-6. -25.,
S. Student housing I have received very few reports for student housing. We need to get
up to date on those reports
9. Student bousing Are the drilled and epvxyed hold downs being inspected in the
Student housing units? Are the holes getting deed enough and ars they gaiting cleaned?
Special inspections list
1. Concrete testing, slump, air, compression tests', Tand 28 day pass/fail.
11. Rebar inspections- location andP asslfail.
3. Masonry rebar and grout- location.
4. Welders certifications.
5. Structural steed bolts, and welds — locatians, pass/fail and corrections.
6. Steed decking welds, studs and. location.
7. Fireproofing thickness and density and location etc.
8. Steam pipe welds inside building and location.
9. Epoxy holes cleaned, mixture, temperature, and location.
10. Dens'
iy isng of soils location and pass/fall under the building.
