The followi'ng comments refer to the house plan referenced above. A.11 held downs shall be installed as
required by the manufacturer.
1. Provide Simpson STHD14 hold downs each side of each aage door.
2. Provide Simpson HD5A or Simpson STHD I ORJ each side of�11windows on the main floor where
the distance to the corner from the edge of the window is 4g" or less,,
3. Provide Simpson HD5 or Simpson S THD I ORJ each side of the bay window in the Breakfast
Area on the main Hoar.
4. A11 exterior framed walls are 2 �a 16" O.C. and are shear walls. Provide %2" APA. rated sheathing
with 8d nails at 4" o.c, at panel edges and 12" ox. in the field. Blocking at paned edges i's requixed.
Framed walls that are 9 feet or less maybe nailed at 5" a.c. at panel edges.
5. The interior wall between the garage and fivin1c; space is a shear wall and shall be configured as
desen'bed 1n item 4 above.
6. PrOrViode solid blacking between the bottom chord of the Trusses from the, front of the gairage in dine
with the left walk of the kitchen.

It has come to our attention that your property located at 162 Hidden Valley Road Rexburg, ID
83440 is currently out of compliance. Although You may not be aware, The City of Rexburg
reqUires that all properties receive a Certificate of Occupancy before being occupied. Section 11Q of
the International Residential Code stats "No building or structure shall be Used Or ocCLipied, and no
change iii the existing occupancy classification of a building or structure or portion thereof shall be
mace iint'[I the Building Official has issued'a certificate of occupancy therefore as provided herein.
Issuance of a certificate of occupancy shall not be construed as an approval of a violation of the
provisions of this code or of other ordinances of the jurisdiction." Furthermore, the Rexburg
Building Department files indicate that a Certificate of Occupancy was never issued. The file for the
property located at 162 Hidden Valley Road contains inspection documents that are incomplete;
there are several ❑LIt standing and noncompliant issues.
On September 12, 2008 Building inspector Quintan Owens and myself bode ConlPliance Officer
1VataIie Powell mads an appointment with your renter Cindy Stray to verify whether or not the
outstanding safety issues had [peen resolved. We inspected the property anId found that the
outstanding items listed on the last inspection, dated May 29, 2008, have not been resolved.
Consequently, we ask that within 30 days you make arrangements to have said property inspected
for compliance. [ have provided with this letter a copy of the outstanding items that will need to be
addressed before a Certificate of occupancy will be !SSL[ed for your property. Please contact me as
soon as you have taken actin in regards to this notice, We appreciate your cooperation. failure to
comply within the 30 days may result in the Initiation of enforcement proceedings.
