Certification of As -Constructed Condition
Dear Ryan:
Gene Palmer, the developer of the subject subdivisions, retained The Dyer Group, LLC to
perform construction observation services and testing on them and to certify to all agencies and
the City of Rexburg as to their as -constructed condition.
Attached is a copy of the as -constructed drawing for the subdivision improvements, along with
copies of bacteriological tests certifying the absence of bacteria in the completed water system
piping both for Phase 3 and Phase 4.
You may recall the original design was prepared under the professional seal of Brent Harris with
drawings prepared by Boyd Cardon. We came into the project during initial construction and were
asked to help modify the design and assume the role of construction observation. Based on a
request by the developer, we contacted Willie Teuscher of your office concerning a modification
of the sewer design to be able to connect it at Marigold Street rather than Lilac Street. This
westerly extension of the sewer line will facilitate future development, and the deeper depth of the
existing sewer line in Marigold Street provided improved slope through much of the system.
Additionally, Phase 4 of the development was approved by the City which required a simple
extension of existing water and sewer lines for approximately 300 feet to serve the additional four
lots that were created. Again, conversations with your office indicated that documenting this on
the As-Builts would suffice.
We hereby certify as to the as -constructed condition of these two phases of the Hidden Valley
Subdivision addition to the City of Rexburg and that all improvements have been constructed,
tested, and accepted in accordance with prevailing public works and health standards of both the
State of Idaho and the City of Rexburg.
Upon review and approval of this record, the developer respectfully requests that you contact
Cindy Johnson of the District 7 Health Department in Rexburg (356-3239) so that she will be in a
position to sign the final plats for these two phases of development.
