Re: Hidden Valley Subdivision Phase 3
Dear Greg;
We have reviewed the plans and overall development proposed for the Hidden Valley Subdivision
and have determined that the city has adequate capacity in our water systems and wastewater system
to meet the demands resulting from the development of this subdivision. The developer will be
paying connection fees that will be used for the expansion of our systems capacities.
If you have questions or need further information please contact me at any time.
Sincerely,
John W. MillarP.E./L.S.

Re: Hidden Valley Subdivision Planned Unit Development Preliminary Plat Review, to
the City of Rexburg, Madison County;
Dear Gene:
We have reviewed the preliminary plat for Hidden Valley Subdivision Phase 3. The plat appears
to meet state standards, but we, at the Department of Environmental Quality (DEQ), recommend
the following:
1.) A complete set of plans and specifications, for design of the water and wastewater
systems must be submitted to this office. The plans must show service connections
to each lot.
2.) Upon submittal of the pre -construction plans and specifications, we will require
submittal of the applicable DEQ checklists. If the Project Engineer does not have
these documents, please do not hesitate to call this office at the number below, such
that we, at the DEQ, can supply you with them.
3.) This office receives will -serve letters from the City of Rexburg Water and
Wastewater Systems for the increased loads and demands.
A copy of the preliminary plats will be kept on file at this office. If you have any questions
regarding this letter or if we can be of any further assistance, please call me at (208) 528-2650.

Re: Hidden Valley Subdivision, Phase HI, Water and Sewer System Review,
to the City of Rexburg, Madison County; DEQ #: 01-05-33
Dear Gene:
Dirk Kempthorne, Governor
C. Stephen Allred, Director
We have reviewed the water and sewer plans for the Hidden Valley Subdivision, Phase III. The
plans appear to meet state standards and are hereby approved with the following conditions:
1.) As per a conversation I had with Brent Harris on June 25, 2001, correct the drafting
errors for drinking water and wastewater service to Lots 7, 15, & 16 of Phase III. As
well as the Manhole #I elevation corrections.
2.) Maintain the necessary eighteen -inch (18") vertical separation between potable and non -
potable water lines.
3.) Maintain the necessary ten -foot (10') horizontal separation between potable and non -
potable water lines.
4.) This office received a will -serve letter from the City of Rexburg on June 19, 2001. Thus
satisfying the right to connect to an existing system requirement.
This approval is for the system design only. We will give final approval after it is established
that the system is operating properly. Deviations from the approved plans, which might result in
violations of Idaho Regulations, must be reviewed by this office before field installation. This
office must be kept informed of the scheduling of all construction phases so that we can make
spot inspections as necessary.
This approval will expire on June 22, 2002. If construction is not complete by the expiration
date, plans will need to be resubmitted for approval before further construction proceeds. Within
30 days after construction, a professional engineer registered with the state of Idaho must
provide this office with as -built plans or a letter of certification stating that the project was
installed with substantial accordance to the approved plans.
A copy of the approved plans and specifications will be kept on file at this office. Please
reference DEQ # 01-05-33 when submitting any future correspondence related to this project. If
you have any questions regarding this letter or if we can be of any further assistance, please call
me at (208) 528-2650.
Sincerely,
