Please take note of the recommendations provided within this report. The following objects were inspected at the time of this visit.- Year isit:Year Built Type of object Jurisdictional number 2007 � FT High Pressure Steam boiler 2007 � FT High Pressure Steam Boiler 2007 � FT High Pressure Steam Boiler Activities & Observations ID022096 I laa22oss The following conditions were noted during the inspection of this equipment: Manufacture Hurst Hurst Murat The iD022096, Ip022097, and ID42209$, Hurst high pressure steam boilers were inspected externally for compliance with Idaho Boiler Code and the adapted ASME Codes, ASME Section I, Power Boilers and ASMI Code 1331.1, Power Piping. I have included excerpts from the ASME Code Section I, Rules for Construction of Power Boilers and ASME Code B 31.1, Power Piping. The excerpts are directly related to the subject of boiler external piping, piping components, valves and the requirements for materials, design, fabrication, installation, and testing of power piping. The ID022096, ID022097, and ID022098, Hurst high-pressure steam boilers. have vales and piping installed an the main steam line that does not meet the ASME Code Section I, PG 5$,3, PG58.3.2 and ASME Code for Power Piping, 831. 1. The valves and piping including for the free-flow vent most meet these codes to satisfy the Idaho Boiler Code. ASME Section I Power Bolters PG -58.3 boiler External Pi in -q: The following defines the Code Jurisdictional L�Mits at the boiler external piping systems, including general requirements, inspection. The limits are also shown in Figs. PG58.3.1 and PG -58.3.2_ The materiels, de installation, and testing shah be in accordance with ASME B31 ,i, Power Piping. valves, and �i n, fabrication, ASME Section i Fewer Boilers, PG -58.3.2 Two or More Boilers Common Stearn Header: When two or more boilers are connected to a common steam header, or when a single boiler is connected to a header having another steam sourcee.g., a turbine extraction lige}; the connection from each boiler having a manhole opening shall be fifted with two stop valves having an ample free -blow drain between them. The boiler external piping includes SII piping from the bailer proper up to and including the second stop valve and the free -blow drain valve. The 1D022096, 1D022097, and 0022098, Hurst high-pressure steam boilers, valves and piping installed on the main steam line have not been hydrostatically tested as a completed bailer unit. This test shall be conducted in accordance with the following requirements: The ASME Cade Section 1 PG -99 The testing must be competed by the folder of an ASME Code Stamp symbol A, 5, or PP and witnessed by their Authorized Inspector. Ref: PG -99. Zurich Services Corporation Risk Engineering Risk improvement Report ASME, Section l Power Bailers PG -99 Hydrostatic TestingLof the completed boiler unit shalf be conducted inaccordance with the fallowing requirements. After a boiler has been completed (see PG -1 Q4), it shall be subjected to pressure tests using water at not less than ambient temperature, but in no case less tharl 70°F (200C). Where required test pressures are specified in this paragraph, whether minimum or maximum pressures, they apply to the highest point of the boiler system. When the bailer is completed ire the Manufacturer's shop without boiler external piping, subsequent hydrostatic testing of the bolter external piping shall be the responsibility of any holder a valid «5 „ "Al" or <<PPJr stam The mechanically assembled bolter external prying which includes the valves, piping components and p(ping installed on the main steam line for the ID022096, ID422097, and ID022098, Hurst high pressure M steam boilers. These components must be documented on the forms required by ASE Code Section i, PG fi 09.4, PG 112.2-5. ASME Section I Power Boilers PG -109. Note That The Res ansibili for Mechanically assembled bolter external piping which contains no pressure boundary welds does not require stamping, and as such may he assembled by a non- stamp holder. for documentation and hydrostatic testing of a mechanically assembled boiler external piping must be assumed by a holder of a valid "5," "A." or "PP" stamp. ASME Section 1 RG -112.2.5 Form P -4A Manufacturer's Data Report for Fabricated Piping, shall be used to record all shop or field -welded bolter external piping that falls within the scope of this Section but is not famished by the boiler Manufacturer. Form P-413, Manufacturer's Data Report for Field -Installed Mechanically Assembled piping, shall be used to record all field -installed mechanically assembled boiler external piping. Form P -41B shall be used only for piping that contains no joints brazed or welded by the Meld installer. The following excerpt is from the ,preamble of ASME Code B31.1, which details what is required under that code. This code follows ASME Section I, as related in the paragraph below. ASME B31.1, Power Pil2inq, 100.1.'1 This Cede prescribes requirements far the design, materials, fabrication, erection, test, and inspection of piping systems. Piping as used in thEs Code includes pipe, flanges, bolting, gaskets, vales, relief devices, fittings, and the pressure containing portions of other piping components, whether manufactured in accordance with Standards listed in Table 126.1 or specially designed. It also includes hangers, supports, and other equipment items necessary to prevent overstressing the pressure containing components. Riles governing piping for miscellaneous appurtenances, such as water columns, remote water level indicators, pressure gages, gage glasses etc., are included within the scope of this Code, but the requirements for bailer appurtenances shall be in accordance with Section I of the ASME Boiler and Pressure Vessel Code, PG -60. Recommendations made herein are offered to correct these deficiencies and are listed below. Corrective action should be addressed prior to the due date noted for the next inspection visit. Please indicate any action takes on these recommendations by returning the report enclosed with this letter or by calling me at the number listed below. Please be advised that there are currently three outstanding recommendations requiring your action and they are found listed below. Based upon this visit, a report of my inspection activity is being forwarded to the governing jurisdiction. That report is requesting that new certificates of operation be issued for the above-mentioned objects be withheld. If there are Code Violations reported from this visit, they are noted in the recommendation portion of this report. A certificate of operation for that equipment cannot be requested until repairs to correct those conditions are made. These recommendations also incorporate your organizations management controls regarding lass prevention and human element If management controls ars absent or not enforced the probability of having an uncontrolled exposure is much higher. 09-01-07 - Boder - General Maintenance: External Piping Design, Fabrication,Installation, and Testing Open The fQ022096, fD022097, and 1p022098, Hurst high-pressure steam boners, main steam header from the boiller proper up to and including the second stop valve and the free -blow drain valve. Shall have the piping, piping components, and valves must meet the following codes for the materials, design, fabrication, installation, and testing shall be in accordance with ASME Code Section I Rules of construction of Power Bo i lers , and ASME Code B31.I, Power piping. 09-01-42 - Boiler - General Maintenance; Hydrostatic Test of Completed Bailers and External Piping open The ID022096, iD022097, and ID022098, Hurst high pressure steam boilers, valves and piping installed on the main steam line must be hydrostatically tested as required bY the ASME Code Section I dor the construction of power boilers. This test shail be conducted in accordance with the fallowing requirements: each boiler shall be subjected to pressure tests using water at not less than ambient temperature, but in no case less than ?0°F (200C) and 1.5 times the maximum allowable working pressure. Where required test pressures are specified in this paragraph, whether minimum or maximum pressures, they apply to the highest point of the boiler system. The testing must be completed by the holder of an ASME code, A, S, or PP Sump symbol and witnessed by their Authorized Inspector. 09-01-03 -Boiler -General Maintenance. Mechanically Assembled External Piping Lr3ocativn Open The field -installed mechanically assembled boiler external piping. shall be recorded on the Form P -4B (PG1 12.2.5). This form shall be used only for piping that contains no faints brazed or welded by the field installer. Al! documentation and hydrostatic testing of a mechanically assembled boiler external piping must be assumed by a holder of a valid "S," "A." or "PP" stamp.
