Idem I ; Portions of the property shown on the
site survey sheet AS 10 1 at the east end of Main
Street, is not owned by the Hospital. Some of
this propertyis owned by the City of Rexburg as
dart of the original town site or part of the Smith
Park property. This needs to be corrected prior
to construction.
Item 2. At the corner of Millhollow and Morgan
where the curb and gutter are to be replaced,
there is the existing crass drain that will need to
be partially replaced.
Item 3: The plans are not clear as to whether or
not the curb and gutter along Morgan and Reed
is to be removed and replaced. This is show
as ADD ALT 1 and is not clear whether or not
this is included in the project.
Item 4.- The plan shows the removal of the
existing 4 -inch water line an Morgan, this is not
typically done, but we have no problem with it
being done.
Item 5: There are no details shown an how the
water and sewer line crossings are to be
constructed to conform to State standards.
Item 6: A water valve will be required on the
existing six inch line where the new water dine
connects to the existing water line at the corner
of Reed and 4th East.
Item 7: At the very west side of the existing
parking lot where storm drainage is to be
modified adjacent to Ash Ave, it is shown that a
2 inch (orifice) plate will be used. Orifice plates
this small are very prone to plugging and
maintenance prodedures will need to be
documented on how plugging will he monitored
and prevented.

Item 8: The proposed water line layout sums
very circuitous and expensive and leaves
numerous dead fire hydrant lines up to two
hundred feet long. These are not conducive to
good fire flow and have a serious potential for
allowing contamination. We request that the
system be reviewed to decrease the length of
these fire hydrant lead dines or to eliminate them
where possible. A possible re-routing
alternative is attached dor consideration.
Item 9: The fire hydrant detail shows a
minimum depth of four feet; our standards
require that water lines be buried to a depth of
five feet.
Item 10: The conditional use permit requires
that the development along Morgan and Reed
Street be constructed in early summer of 2006.
The proposed storm water pollution prevention
plan shows the silt fencing being placed
adjacent to the curb and gutter along Morgan
and Reed. It is anticipated that the
development of the landscape area and
replacement of curbed gutter and sidewalk
Tong Morgan Road will take place verb early in
the project. The silt fencing will be required
during the grading and palling of the parking lot
and the development of the storm water
system. It will be required that two slit fences
need to be constructed. One along Reed and
Morgan which will be a relatively short duration,
and another one will be located south of the
proposed sidewalk and landscaped area that
will need to remain in effect until the site work is
cQmpieted.
Item 11: The storm water prevention S'WPPP
on sheet C-501 shows a detail of the silk fence
section; however, there is insufficient detail to
determine how this is to be constructed, of
what materials, to what height, and other

Item 12: Sheet C-502 has a note that requires
a notice of intent be filed with the environmental
protection agency. Has this notice of intent
been filed? A copy of correspondence with
LPA needs to be provided to the City. Also,
sheet L-101 shows the landscaping area to
done slang Reed and Morgan. The pian shows
the area to be finished with lawn; however,
there is no clarification whether or not this is to
be sod, seeded or other methods. We reed
clarification on this item. It is anticipated that
with, commitments made to the adjacent
property owners that sad would be utilized in
the landscaping area.
Item. 13. Sheet ES -101 shows the site electrical
plan and where light fixtures are to be placed.
We have not received details, as required by
the development code, an the type of fixtures
that are to be used. Catalog cuts and other
details will need to be provided for review of the
lighting plan design. The lighting plan design
submitted for this project should show lighting
intensitities in the parking lots and also at the
property boundaries around the project site.

by the Idaho Department of Environmental in
Idaho Falls, Idaho.
