services on the above referenced 0 pr Q1 ect.

estimate also based on a presumed construction schedule. If the construct -ion schedule does not follow
I`vlU's estimated Production rates and schedule, the qUantities and number of trips required will vary. Please
review the number of trips and quantities to insure that each item follows your construction schedul e/pro duct ion
rates. Variances from our estimate and your schedule/production rates will be billed at the indicated unit rates.
The result may be a decrease or increase in the total testing and inspection costs. Scope of services for the
project have been outlined below:

A detailed explanatiorl
of testinglznspection items for each phase of construction has been. provided below:
• Soils -Fill Placement monitoring and compaction testing will be. Performed dry others.
• Concrete - "Special Inspectian'' of concrete is required. 5trucfuxal concrete wi.1.1 be tested at intervals
of one set of 4 concrete cylinders for each 150 cubic yards or fraction thereof of each concrete rnix
placed each day.
• Masonry - "Special Inspectian" of masonry is required. Inspection will consi"St of -icy cell

condifions and reinforcing steel placement prior to grouting and witnessi r naDf 14 routing activities dur�ng

Structural Steel -- "Special Inspection" of welded and/or bolted corvleciiUns zs rec�u�red. Periodic
visual welding inspection will be performed on all field welds. Bolted cormeetinns will be inspectedi"11
accordance with the project specifications. The fiat trips should be performed at the start -t -up of welding
or bolting operations to check certifications or bolt calibrations and review procedures with an -site
welders 1 steel workers. Intermediate trips should be perfon-ned to inspect welds r bolts that are to be
covered prior to the. final inspection. The final inspection should be performed'at the completion of
welding and 1 or bolting'. but prior to departure of the steel crew, to inspect the finished product.

Inspected reinforcing steel placement at shear wall from gridline 8 to 10 on row G, elevation 8'8". to 13 ' 4"; and

Performed a visual inspection of reinforcing steel, concrete masonry units and mortar, and inspection of how -lift
and hl"gh-lift grout at locations noted above. A total of 6 cubic yards of Walters Ready Mix gout min #42

STRUCTURAL MASONRY INSPECTION REPORT
On the above date, our rep�esexatative performed work on the referenced project as reported below.
inspected reinforcing steel placement at staircase on both west and east side, elevation 1 6' to 20'; and elevator,
elevation 16' to 24'.
Performed a visual inspection of reinforcing steel, concrete masonry units and mortar, and inspection of low -lift
grout at locations noted above. A total of 8 Cubic yards of Walters 7 -bag grout mix was placed at the above
location and was mechanically vibrated.
Note: Contractor notified on mortar bedding, joints were not sufficient and need repair and re -inspection.
Contractor Bart (Sommer Builders)vas informed by MTI representative that due to lark of 100% bond
of mortar, that an adhesive banding; material would be needed to bond existing hardened mortar with
replacement mortar. Slump of grout measured 10'/4 ".

On the above date, our representative performed work on the referenced project as reported below.
Inspected reinforcing steel placement at (1) shear wall on row B from gridline 8 to 10, elevation 0' to 4';
(Z) stair well on gridline I.5 from raw D.2 to E.4; an D.2 from gridline I to 1.5; on gridline 1 from D.2 to E.4;
on E.4 from gridline 1 to 1.5, elevation 1T' to 16'; and (3) exterior wall on row C from gridline 3.9 to 8.5; on
gridline 4.6 from C to B; on row B from gridline 4-:6 to 7.3; and on gridline 7.3 from B to C., elevation S' to 13'.
Performed a visual inspection of reinforcing steed, concrete masonry units and mortar, and inspection of law -lift
grout at locations noted above. A total of 10 cubic yards of W'alters �4rout mix #516 urns placed at the above
location and was mechanically vibrated. Slump = 10`10
Work inspected was in compliance with project plans and specifications.

lift);
and at west stairwell from gridline 1 to 1.5 between D.4 and E.4, elevation S to 12' (haw -lift}.
Performed a visual inspection of reinforcing steel, concrete masonry units and mortar, and inspection of Low -lift
and high -lift grout at locations noted above. A total of 13 cubic yards of Walters grout mix 2RC 0p -Class
40-7V2 ib. was placed at the above location and was mechanically vibrated.
Node: Inspector used a flashlight to inspect bottom of cells on high -lift.
Work inspected was in compliance with project plans and specifications.

STRUCTURAL MASONRY INSPECTION REPORT
Phase II, Rexburg
On the above date, our representative performed work on the referenced project as reported below.
Inspected reinforcing steel and block placement from gridline 2 to 1 on row G, shear wall, elevation $' to 13 '4"
(high -lift); from gridline 1 to 1.5 between rows D.4 and E.4, stairwell, elevation 4' to S' (low -lift); and from
gridline 5.5 to 6.5 between F.4 and F.8, elevator shaft, elevation 4' to 12' (high -lift).
Performed a visual inspection of reinforcing steel, concrete masonry units and mortar, and inspection of low -lift
and high -lift grout at locations noted above. A. total of T/2 cubic yards of Walters grout mix 2RCOO Class 40
was placed at the above location and was mechanically vibrated.
Node: Shear walk was inspected with a flashlight and cleanouts were provided fir the elevator shaft. Some of
the rainwater collected on the 2'-'d deck and drained into the shear vva11. Contractor knocked out a hole in
the bottom cell; no appreciable amount of water exited the hale. Contract -or has been notified to cover
unused block.

Arrived ars site and inspected welders' qualification papers; they were current. Checked. procedure
qualifications. Visually inspected the completed welding; performed by qualified welders L15II1g a SMAW
process with E70.;2.4 and E7018 electrodes and verified proper electrode storage. Inspection consisted of sincyle
pass fillet welds. Type of connections and locations: joist — wide beam, all joists from gridline 7 west, on row
E from gridline 7 to 8, and south of row F from gridline 9 to 10.

On the above dale, our representative perfonned work an the referenced project as reported below.

ngs, Area C from row G to B.4 at gridline 11 and on row A at
gridline 9.7.
Performed one air entrainment test and one slump test. Observed placement and consolidation of 16 pubic
yards of Walters concrete mix #1.9+0 placed at locations noted above, and cast one set of four cylinders.
Cylinders: #62232-#62235
Note: Air spec 3% f 1.5% - tested at 6.2%,; contractor notified.
Work inspected was in compliance with project plates and specifications, except as noted.
