To Whom it may concern:
Great Basin Services, Inc has removed aprox 1500 sq fl of Asbestos floor tiles and mastic
located at the approach to the bowling lanes. This is the asbestos materials identified in the
Asbestos inspection report. These materials were removed using a full negative air
containment with hand removal methods. All work was done by trained and certified asbestos
workers using EPA approved removal methods. All materials removed were taken to the
Bannock County Landfill for disposal, which is an EPA approved landfill that accepts asbestos
waste. A Signed landfill manifest have been provided to show proper disposal as required.

Introduction
On 2-15-2021, accredited EPA Asbestos Inspector, Dave Price, of Great Basin Services, Inc., conducted a
site inspection and collected samples of suspect Asbestos Containing Building Materials from the building located at
585 North V East, Rexburg, Idaho. This building is a bowling center. The purpose of this inspection and sample
collection was to identify and locate any possible asbestos containing materials that might be present in the building
prior to Demolition/burning. The suspect ACBM samples were cataloged and uniquely labeled onsite and then
submitted under standard chain -of -custody (COC) protocol to EMSL Analytical, a National Voluntary laboratory
Accreditation Program (NVLAP) accredited laboratory for analysis of materials using Polarized Light Microscopy
(PLM), EPA/600/R-93/116 methodology. This PLM method is standard for Asbestos Bulk testing. This inspection
was an initial inspection and sampling. Because of the snow on the roof, more sampling may be need when the
entire roof is visible.
Background
The bowling alley has been partially demolished inside, The ceiling tile and drywall over the lanes has been
removed and most of the insulation and ceiling tile still remain on the floor. The insulation in this area was fiberglass
bats and the ceiling was exposed up to the plywood decking. There was floor file located at the start of the lanes.
This area had 2 different layers of tile, with several colors on the bottom layer. The area at the start of the lanes was
the only place floor tile was found. However, there was sheet vinyl flooring in the bathrooms.
The front half of the building had offices, rooms, bathrooms and a bar that all had drywall with texture on
the walls. On the ceiling in these areas there was spray on texture over the drywall.
The Insulations found in the building were all fiberglass. We did sample some white styrofoam looking
insulations from the exterior cinder block wall cavities.
All of the exterior walls were of cinder block, and the roofing components were of typical wood
construction methods.
As indicated above, because of snow on the roof, we were not able to inspect the entire roof for mastics or
other roofing materials. We were able to inspect and sample the bottom eve of the front ends of the building It
appears that there is only wood decking, with metal over that. The complete roof will need to be inspected once the
snow has melted, just to make sure. This may require further sampling.
Sampling
Dave Price, accredited Asbestos inspector, collected a total of 21 original PLM samples of suspect
Asbestos Containing Building Materials (ACBM) from the building and trailer houses. These 21 samples were
analyzed, at the lab using the PLM/600/R-93/116 methodology and broke down into a total of 43 individual
materials that were analyzed separately. All of the original samples were of materials that were identified as
possible ACM.
in this sampling scheme, sampling locations and number of samples were selected so that they were
representive of the homogeneous sampling areas with regard to the material present and It's potential asbestos
content. The sampling scheme was designed to be comprehensive and intended to identity ACM that might be
disturbed during Demolition/ Renovation activities. The initial testing method used is a Polarized Light
Microscopy ( PLM) method. This Is an EPA accepted method for Asbestos testing. ACM ,under current regulation,
is defined as material with an asbestos content of Greater than 19/0. Any sample analyzed by "polarized light
microscopy" ( PLM) method, as containing "Greater than 1 % Asbestos", will need to be re -analyzed using an
EPA 400 point count method with Gravimetric Reduction, or be assumed to be ACM. The EPA 400 point count is
a more precise and accurate method of analyzing a sample for asbestos. in some cases, point counting can provide a
lower percentage of asbestos Ina sample, to an amount of asbestos under the EPA threshold amount. Point
counting a sample will not always reflect a lower percentage of asbestos. The type of materials tested, and the
percentage of asbestos found using the PLM method, can affect the point count results.
All samples were sent to EMSL Analytical, San Leandro, California.

Conclusions
The following identifies the sample Numbers, sample materials tested, and location of sample. Out of the 43
individual PLM sample results. 4 Individual sample results indicated the that asbestos was present In the amounts
or "Greater than I%". which by EPA rule, are considered to be Asbestos Containiast Materials. Ifany
material is found to contain 1% or less asbestos, by EPA role, the sample is considered to be non -asbestos materials
and can be disposed of as general construction debris. The following are the 4 samples that tested above EPA
regulated amounts:
1. FT -03 -Mastic -1 Q 2% asbestos. This is the black mastic on the bottom of the red floor tile on the lower concrete
approach of the lanes. This sample was taken on the south side of the building
2. FT -04 -Mastic -2 Q 2% asbestos. This is the black mastic on the bottom of the orange floor tile on the lower
concrete approach of the lanes. The orange layer is the top layer in this ares. This sample was taken on the south side
of the building
3. FT -05 -Mastic -2 Q 4% asbestos. This is the black mastic on the bottom of the white floor tile on the lower
concrete approach of the lanes. The white tile is the bottom layer of flooring In this are&. This sample was taken on
the North side of the building
4. PT -06 -B -Mastic 2 Q 2%a This is the black mastic on the bottom of the Red floor tile on the lower concrete
approach of the lanes. The red the is the bottom layer of flooring in this area This sample was taken on the North
side of the building
'( there were 2 samples of sheet vinyl flooring in the men and women's bathrooms that tested at <1% asbestos
but because they are under the EPA threshold amount, removal is not needed before demolition. Ibis material
can be disposed of as general construction debris)
-All of the black mastic under the floor tile and any floor tile that has black mastic on it is considered to be Asbestos
Containing Material ( ACM). These are both under a non asbestos layer of the and glue. Ifthe top layer of the can be
separated from the bottom layer, it can be disposed of as non- asbestos waste and can remain on site.
-Estimated sq It of the floor tile and mastic is 1850 sq ft of surface area that would need to be removed. The location
or the floor tile is at the start of the lanes to the raised floor, and from the North end of the building to the south end of
the building.
EPA does state that floor tile can remain during demolition as long as they are not made to become friable
airborne) using demolition methods. However, most landfills will not accept debris with any asbestos materials in
them. Our recommendation is to remove these asbestos materials before demo. Because this is a commercial building,
removal of asbestos materials should only be done by trained and certified asbestos workers using approved EPA
removal methods. The materials removed are required by EPA to be disposed of at an approved landfill that accepts
asbestos.
Because this is a commercial building, a 10 day EPA notification for the asbestos removal and also the
demolition is required. These notifications can be done by the individual abatement companies or Demolition
companies, or a combination of each. See the attached copy of a typical EPA 10 day Notification.

Great Basin Services, Inc has re -inspected the exterior of the roof. On the initial
inspection the roof was covered with aprox 10 inches of snow. After the re -inspection was
completed, there were no new materials found on the roof, only the same materials as previously
sampled, on the initial inspection. This should information should be added to the initial report.
