WEATHERSTON being first duly sworn, depose and say, that I am the printer of THE STANDARD -JOURNAL, a semi- weekly newspaper printed and published semi-weekly at Rexburq, Madison County, Idaho; and I do solemnly swear that the INVITATION TO BID - CITY OF REXBURG a copy of which notice appears attached hereto, was published in the regular and entire issue of said newspaper, and not in any supplement issue thereof for THREE consecutive issues, the first publication having been made on APRIL 17, 1997 and the last publication having been made on MAY 1, 1997 Further, that said newspaper is one of general circulation in said county; that it is a legal newspaper, conforming to legal specifications under the Idaho Statutes; that it has at least two hundred bona fide subscribers living within the county of publication; and that it has been continuously and uninterruptedly published in said county during a period of seventy eight consecutive weeks prior to the first publication of the said notice. COUNTY OF MADISON j On this 2TH day of MAY in the year of 1997 , before me, a Notary Public, Personally appeared Terry Weatherston, known or identified to me to be the person whose name subscribed to the within instrument, and being by me first duly sworn, declared that the statements therein are true, and acknowledged to me that he executed the same. ,, Res'iGffng at Rexburg, Idaho My commission expires: May 21, 1999 INVITATION TO BID Separate sealed bids for the construc- tion of a 1,500,000 gallon concrete water storage reservoir, well pump revisions, booster pump station, and related work will be received by the City of Rexburg at the office of City Hall, 12 North Center, Rexburg, ID 83440 until 3 p.m. on May 2, 1997, then at said office publicly opened and read aloud shortly thereafter. The CONTRACT DOCUMENTS may be examined at the VIOw1910ca(ions: Forsgren Associates, Inc., 350 North 2nd East, Rexburg, ID 83440. Associated General Contractors, 185 S. Capital, Idaho Falls, ID 83402. City of Rexburg, 12 North Center, Rex- burg, ID 83440. FW Dodge/Intermountain Contractor, 110 N 27th Street, Boise, ID 83702. Copies of the CONTRACT DOCUMENTS may be obtained at the of- fice of the Engineer located at 350 North 2nd East, P.O. Box 549, Rexburg, Idaho 83440 upon payment of $50 for each set, none of which will be refunded. A five per- cent (5%) bid bond will be required. Each proposal must be submitted on1 the prescribed form and accompanied by a certified cashier's check or a corporate bid bond executed on the prescribed form, payable to the City of Rexburg, Idaho, in an amount not less than five per- cent (5%) of the amount bid. The suc- cessful bidder will be required to furnish performance and payment bonds, each in the amount not less than 100% of the contract price. Any contract awarded under this Invita- tion to Bid is expected to be funded from HUD. Neither the United States or any of its departments, agencies or employees is or will be a party to this Invitation to Bid or any resulting contract. Before a con- tract will be awarded, compliance with all HUD requirements specified in the Con- tract Documents will be subject to the fun- ding agencies review and approval. Bidders on this work will be required to comply with the President's Executive Order No .11246 as amended 12138, 12432 and 11625. The requirements for bidders and contractors under these orders are explained in the Contract Documents.
