Main (PO Box 280) Phone: 208-359-3020 x2 Rexburg, Idaho 83440 Fax: 208-359-3024 www.rexburg.org comdev@rexburg.org Conditional Use Permit The Madison Memorial Hospital having come before the Rexburg Planning and Zoning Commission on July 7, 2005 for the purpose of obtaining permission to expand the existing facilities. The requested permission was granted upon the following conditions: 1. Prior to construction beginning, the Hospital must submit a construction schedule to the City of Rexburg Community Development department. 2. There will be no direct vehicular traffic access to Morgan Drive. 3. The services of a professional landscape architect will be used in developing and constructing the proposed and agreed upon 37 foot landscape buffer, with attention to protecting the neighbor's privacy. 4. The existing landscape buffer along Millhollow Drive shall be reviewed and attention given to bringing it into compliance with a similar standard as has been agreed to with respect to the Morgan Drive buffer. 5. That mature trees be used in completing the new landscape buffer, and that prior to occupancy of the new portions of the building, the buffer shall be in place in accordance with the agreed upon terms with the adjoining property owners association. 6. During all phases of construction, there shall be in place sufficient fencing and screening to provide for a debris and visual buffer. 7. The Hospital shall hold public meetings on a regular basis to keep interested parties advised as to issues and respond to reasonable questions concerning the construction project. 8. Proof of acquisition shall be provided to the City prior to construction beginning. 9. A plan for accessing all properties interrelated properties, which plan shall not include access from Morgan Drive. 10.That the Hospital provide the City with a transportation study addressing the traffic patterns associated with hospital traffic. 11.That the standard conditions of 6.13 B in the Development Code be applied. Failure to comply with the above stated terms and conditions may result in the forfeiture of the permit, and revocation of all rights associated with the issuance of this permit.
