ADA Ramp Inspection Definitions
Location Type - Check "Corner' if the ramp is located at an intersection with a street, alley or major driveway. Check "Mid -block"
if the ramp is located at a mid -block crossing or at the mid -block end of a sidewalk.
Corner or Side - For corner locations, enter the corner on which the ramp is located. For mid -block locations, enter the side of
the state highway that the ramp is located on. Use the cardinal direction.
Park Strip - Check the box if a park strip or planter area is present adjacent to the ramp along the state route.
Ramp Type - Enter the detail number of the ramp installed at this location. For details modified in the field, insert "(mod)" after the
detail number. For example: Al 8a (mod).
Number of Ramped Areas - See the diagrams below: If there are two perpendicular ramps on the corner, the number of ramped
areas would be two. If there are two parallel ramps on the corner the number of ramped areas would be four.

Ramp Direction - The direction that most closely parallels the direction of pedestrian travel on that ramped area.
Ramp Width - Measured at the narrowest point; must be at least 48 inches minimum for acceptance.
Running Slope - Measured parallel to the direction of pedestrian travel; no more than 8.3% maximum for acceptance.
Cross Slope - Measured perpendicular to direction of pedestrian travel; no more than 2.0% maximum for acceptance.
Flare Slopes A and B - Measured parallel to the curb and gutter; no more than 10.0% maximum for acceptance if they are
located in the pedestrian circulation path. If no flares on the ramp, enter N/A.
Discontinuities - Select Yes if surface discontinuities greater than ''/z' are present on the ramp or landing areas. There can be
no surface discontinuities for ramp acceptance.
Landing Details - A landing must be provided and be at least 4'x4' minimum for ramp acceptance.
Landing Width - If less than 4'x4', record the ramp width in inches. If greater than 4'x4', leave this field blank.
Landing Depth - If less than 4'x4', record the ramp depth in inches. If greater than 4'x4', leave this field blank.
Maximum Slope - Record the maximum slope in any direction; no more than 2.0% maximum for ramp acceptance.
Detectable Warning Details
Truncated Dome Type - Cast -in-place domes are inlaid into the wet concrete and include material types such as polymer
composite, concrete composite, polymer tiles and cast iron. Surface applied domes are installed on the surface of the concrete
and include surface applied polymer composite.
Detectable Warning Depth - Enter the depth of the detectable warning surface in the direction of pedestrian travel (front to
back) in inches; must be at least 24 inches minimum for ramp acceptance.
Detectable Warning Spans Curb Cut - Must span the entire width of the ramp surface for ramp acceptance.
Within 8" of the Curb Line (face of curb) - Check Yes if at least one corner of the leading edge of the detectable warning
surface is within 8" of the curb line; at least one corner of the leading edge must be within 8" of the curb line for ramp
acceptance.

Dome Dimensions are Correct - Dome dimensions are shown in the Standard Drawings.
Transition Details
Lip Present - Check Yes if a lip greater than ''/z' is present in the transition area between the ramp and the roadway. This could
be at the flowline or where the edge of the gutter meets the roadway. There can be no lips for ramp acceptance.
Gutter Counter Slope - Measured perpendicular to the flowline, no more than 5.0% maximum for ramp acceptance.
Gutter Flowline Slope at Ramp - No more than 2.0% maximum for ramp acceptance.
Pedestrian Crossing Details
Marked Crosswalk - This could be across the state highway, intersecting roadway, or both.
Crosswalk Clear Space - Check Yes if there is a 4'x4' minimum clear space within the crosswalk area. If there is no marked
crosswalk, check Yes. A clear space must be provided within the crosswalk, if a crosswalk is present, for ramp acceptance.
Pushbutton Height within 15" to 48" - If there is no pedestrian pushbutton, leave this field blank. Pushbutton height must be
within 15" to 48" above clear space for ramp acceptance. Height of 42" preferred.
Pushbutton Clear Space - If there is no pedestrian pushbutton, leave this field blank. Check Yes if there is a clear space in
front of each pushbutton for either a forward or parallel approach with 2% maximum slope in all directions. The clear space
shall be located with respect to the pushbutton per the details in the plans for either a forward or parallel approach. Clear space
must be provided in front of pushbuttons for ramp acceptance, except where signal poles would have to be added or relocated.
Ramp Comments
Ramp Comments - Explain any additional issues or comments about the ramp that are not covered by this form. List any
technical infeasibility that prevented any ADA requirements from being met and explain all reasons why.
Ramp Accepted - Check the box if this ramp meets the all ADA requirements for acceptance or has an approved technical
infeasibility.
