This is not a change Order, a construction change directive or a direction to proceed with the work described in the
proposed modifications.
Please submit an itemized proposal for changes in the Contract Sum and Contract Time for proposed modifications to the
Contract Documents described herein. Within Ten (10) days, the Contractor must submit this proposal or notify the
Architect, in writing, of the date on which proposal submission is anticipated.
Description: (Written description of the work)
1.1 Please provide a cost proposal to provide the changes outlined below. This request does not imply acceptance of the
proposal.
Provide all materials and labor to extend Waiting 144 to the North to Grid line B per the attached documents. All finishes
and casework are to match existing and be approved by the Owner prior to ordering.
If this is accepted it will need to be submitted to the City of Rexburg as a change to the Building Permit.

THE PROJECT CONSISTS OF A PARTIAL RENOVATION OF THE EXISTING MEDICAL AND DENTAL CLINIC AND A COMPLETE INTERIOR
RENOVATION, PARTIAL EXTERIOR RENOVATION AND RE-ROOF OF THE ADJACENT BUILDING. THE EXISTING MEDICAL AND DENTAL CLINIC
RENOVATION WILL INCLUDE, ON THE MEDICAL SIDE, RENOVATING TWO OF THE EXISTING OFFICES INTO EXAM ROOMS THAT WOULD INCLUDE
CASEWORK WITH SINKS, RENOVATE THE LAB STORAGE AREA, PROVIDE STORAGE IN THE OLD BREAKROOM AREA AND A CONNECTING
HALLWAY TO THE ADJACENT BUILDING. THE RENOVATION ON THE DENTAL SIDE WILL INCLUDE CONVERTING THREE OF THE EXISTING OFFICES
INTO OPERATORY ROOMS THAT WILL REQUIRE CABINETS WITH SINKS, RELOCATING THE MECHANICAL ROOM TO THE GARAGE, PROVIDE
STORAGE IN THE OLD BREAKROOM AREA AND A CONNECTING HALLWAY TO THE ADJACENT BUILDING. THE RENOVATION WILL ALSO INCLUDE
EXTERIOR IMPROVEMENTS TO THE SOUTH AND NORTH SIDE OF THE BUILDING TO MATCH THE EXISTING EAST ELEVATION.
THE ADJACENT BUILDING WILL INCLUDE A COMPLETE INTERIOR RENOVATION WITH A NEW MENTAL HEALTH SUITE, RESTROOMS, MEDICAL
PROVIDER OFFICES, DENTAL PROVIDER OFFICES, STAFF BREAKROOM, CONFERENCE/MEETING ROOM. THE RENOVATION WILL ALSO INCLUDE
EXTERIOR IMPROVEMENTS TO MATCH THE EXISTING BUILDING WITH A NEW, INVITING ENTRANCE FACING SOUTH AND A COMPLETE ROOF
REPLACEMENT THAT WILL INCLUDE TUBE SKYLIGHTS.

10 POWER FOR MOTORIZED SCREEN; COORDINATE LOCATION AND
CONNECTIONS WITH EQUIPMENT.
11 RECEPTACLE MOUNTED FLUSH IN CEILING FOR OVERHEAD
PROJECTOR, VERIFY EXACT LOCATION WITH OWNER PRIOR TO
ROUGH-IN. INSTALL J-BOX ABOVE CEILING MOUNTED TO STRUCTURE
WITH A 10FT MC CABLE WHIP TO RECEPTACLE.
12 CONNECTION TO AUTOMATIC ADA DOORS; FIELD VERIFY
