IN THE EVENT OF CONFLICT THE ENGINEER SHALL BE NOTIFIED IMMEDIATELY SO CLARIFICATION MAY BE MADE PRIOR TO THE START OF CONSTRUCTION. 3.ELEVATIONS SHOWN ARE 2004 CITY DATUM AND ALL ELEVATIONS AND ARE TO FINISHED SURFACE OR PIPE INVERT UNLESS OTHERWISE NOTED. 4.THE CONTRACTOR SHALL PROVIDE REQUIRED TRAFFIC CONTROL IN ACCORDANCE WITH THE LATEST EDITION OF THE MANUAL ON UNIFORM TRAFFIC CONTROL DEVICES (MUTCD). 5.GENERAL LOCATIONS AND SIZES OF UTILITIES ARE SHOWN ON THE PLANS. THEY ARE TO BE USED FOR GENERAL INFORMATION ONLY. IT IS THE RESPONSIBILITY OF THE CONTRACTOR TO NOTIFY THE APPROPRIATE UTILITY COMPANIES WHEN CONSTRUCTION MIGHT INTERFERE WITH NORMAL OPERATION OF ANY UTILITIES. IT SHALL ALSO BE THE CONTRACTOR'S RESPONSIBILITY TO HAVE THE APPROPRIATE UTILITY COMPANY FIELD LOCATE ANY UTILITY INSTALLATIONS WHICH MIGHT BE AFFECTED BY CONSTRUCTION PRIOR TO BEGINNING WORK IN THAT AREA. THE CONTRACTOR SHALL BE RESPONSIBLE FOR MAINTAINING SERVICE OF EXISTING UTILITIES AND FOR RESTORING ANY UTILITIES DAMAGED DUE TO CONSTRUCTION AT NO ADDITIONAL COST TO THE OWNER. 6.THE CONTRACTOR SHALL BE RESPONSIBLE FOR SECURING SOURCES FOR GRANULAR MATERIAL, DISPOSAL SITES, AND ANY ADDITIONAL MATERIALS THAT MAY BE NECESSARY FOR PROPER CONSTRUCTION OF THIS PROJECT. 7.IT SHALL BE THE RESPONSIBILITY OF THE CONTRACTOR TO MAKE ARRANGEMENTS FOR WATER REQUIRED FOR TESTING, COMPACTION AND DUST CONTROL MEASURES. 8.A CITY OF REXBURG PUBLIC WORKS LICENSE IS REQUIRED FOR ANY/ALL CONTRACTORS WORKING WITHIN THE PUBLIC RIGHT-OF-WAY OR EASEMENT. 9.APPROVAL OF THE CITY ENGINEER IS REQUIRED FOR ANY PROPOSED CONSTRUCTION WITHIN A PUBLIC RIGHT-OF-WAY OR EASEMENT. 10.ALL CONSTRUCTION INCLUDING CONCRETE/ASPHALT PLACED WITHIN THE RIGHT-OF-WAY REQUIRES INSPECTION AND APPROVAL OF THE CITY ENGINEERING DEPARTMENT. THE DEPARTMENT SHALL BE NOTIFIED AT LEAST FOUR (4) HOURS PRIOR TO PLACEMENT (359-3020). 11.A PUBLIC RIGHT-OF-WAY USE PERMIT IS REQUIRED FOR ANY WORK WITHIN ANY PUBLIC RIGHT-OF-WAY OR EASEMENT. THE CITY OF REXBURG ENGINEERING DEPARTMENT SHALL BE NOTIFIED AT LEAST TWO (2) DAYS PRIOR TO ANY EXCAVATION UNDER THIS PERMIT (359-3020). 12.THE CONTRACTOR SHALL RETAIN AND PROTECT OR REMOVE AND REPLACE ALL LANDSCAPING, TREES, UTILITIES, DITCHES, CULVERTS, FENCES, MAILBOXES, SIGNS, LIGHT POLES, HEAD GATES, AND ALL OTHER MISCELLANEOUS ITEMS AS CALLED FOR IN THE PLANS. ANY DAMAGE DONE BY THE CONTRACTOR TO EXISTING STRUCTURES TO REMAIN SHALL BE REPAIRED OR REPLACED AT CONTRACTOR'S EXPENSE. 13.CONTRACTOR TO BE RESPONSIBLE FOR FILING THE OFFICIAL SWPP PLAN AND NOI IN ACCORDANCE WITH EPA STANDARDS AND MAINTAINING THESE STANDARDS INCLUDING STORM WATER POLLUTION AND DUST CONTROL PER THE STATE OF IDAHO CATALOG FOR BEST MANAGEMENT PRACTICES (BMP'S). 14.SITE ADDRESS: ,REXBURG, IDAHO (CITY OF REXBURG TO PROVIDE).
