kitchen sink and laundry; these need firecaulk around drains in drywall and metal
covers over cleanouts. Some cleanouts not found behind dryers and might still be
covered - refer to plumbing insp. unrated boxes in hall ceiling at several
locations. 1102: fc data line in mech closet. 1103 tape or frclk joints in mech
closet clg patch. Utility room not done (incl. beam pockets on sth wall). 1105 fc
drywl patch on mech clst clg. 1109: patch clg in lndry. Smk Det missing in last
bdrm. SD too close to corner in 1st bdrm. 1113 seal tub/shwr perimeters. 1115
SD too close to corner. 1117 SD close to corner. 1114 seal chip at base of tub.
light switch not working in 1st bdrm. East utility penetrations not f.c. 1112 seal
around shwrs. SD too close to supply rgstr. 1110 missing sply rgstr in 1st bdrm.
1108 finish seal around tub. Utility rooms on east & west are not finished for fire
rated penetrations (West utility on 1st floor also has beam pockets not filled with
concrete and needs fire rating). Can Mech. engineer verify why duct penetrations
in Utility "shaft" do not show combination Fire/Smoke dampers and only have
Fire dampers? Not all exit signs are readily visible as required by code. No exit
sign by door in west or east hall (will verify requirements).

Levels 4 & 5: 1511 missing light fixture in mech. closet. 1510 & 1515 seal duct at
connection w/ furnace. 1515 Fan does not work in "on" position. All units: gas
line penetration on side of furnace to be sealed on direct vent furnaces. Cover
still on fire alarm on stair C. 1507 fresh air intake incomplete. Game room: fire
alarm cover on. 1510: ADA vertical grab bar to be 39 - 41" from back wall. shelf
in laundry interferes with clearance. 1508: leak between kitchen & entry. 1401
orange foam not a fire stop (mech closet). 1403 S.D. in 2nd bdrm not connected.
1405 hole in clg in mech closet above door. 1407 Open tube penetrating ceiling
above door in mech closet. Utility rooms behind elevator: FC wall penetrations;
firedamper on 4th floor (2 locations)? 1402: orange foam not a fire stop (mech
room). 1413: missed fire caulk under 3rd vanity. 1417 S.D. not connected
(middle & last bdrm).
Inspector Comments:
Level 3: Utility rooms need non-absorbent surfaces around mop sink. Utility room
by elevator: fire stop hole and duct penetration on wall by door. 1307: Orange
foam is not a fire stop (above door in mech room). 1305 hole in ceiling of mech
room above door. 1303 finish seal around shower in 3rd bathrm. Finish ceiling
patch around sprinkler in laundry. 1312 mech room locked shut (no inspection).
Level 2: 1203 tape on clg. 1205:non-operable window in last bdrm. check ac line
to furnace - found frost on pipe and making noise. North utility: seal around duct
penetrations. 1211 metal cleanout cover needed under sink. 1213 middle tub
needs caulking by door. 1215: seal duct/furnace connection. 1217 S.D. right in
corner (2nd bdrm). 1214: S.D. too close to corner (2nd & 4th bdrm). 1210: install
cover box for wires in furnace. 1312: seal duct/furnace connection.

engineer verify why duct penetrations in Utility "shaft" do not show combination
Fire/Smoke dampers and only have Fire dampers? All units: seal gas line
penetration into direct vent furnace.
Levels 4 & 5: 1511 missing light fixture in mech. closet. 1510 & 1515 seal duct at
connection w/ furnace. 1515 Fan does not work in "on" position. 1507 fresh air
intake incomplete. 1401 orange foam not a fire stop (mech closet). Utility rooms
behind elevator: FC wall penetrations; firedamper on 4th floor (2 locations)?
