Smoke & C.O detector outside bedrooms * Smoke alarm in NW bedroom in unit 202 is
not working.
* Smoke alarm by kitchen in unit 101 is not
working.

All special inspections are completed and
reported.
Has special inspection been done on stair
structure?
FAIL
Mechanical, Electrical, Plumbing are required as
well as P&Z, Public Works, Fire Dept. and Fire
Sprinklers where sprinkler system exists.
Not all finals have been approved FAIL
All egress components (ramps, landings, stairs,
handrails, guardrails, etc.) meet code.
Discussion at beginning of project identified the
requirement for solid risers per 2012 IBC
1009.7.5.3.
* Risers are open

Corridors (including doors) are rated as per
plans and are proper width.
Vent openings in corridor walls from corridor to
equipment rooms require dampers to maintain
fire rating. The one in elevator equipment room
needs fire rating around damper above door.

Doors are correct size, swing, and rating.Fire rated doors shall be self closing (make
adjustments on closers as needed)

Fire rated structural elements are sprayed,
rocked, or covered as per plans.
* Bearing wall left of furnace shall be rated for
VA construction type. Maintain rating around
duct and return air perimeter where open and
at thermostat wire penetration.
* Tape perimeter around opening to roof access
(roof assembly is one hour rated)
* Fire rating to be maintained at plastic clean
outs under kitchen sink in 301,302, & 304
* Fire sprinkler missing escutcheon in 3rd level
common area and in bedroom of 103
* Data room (3rd floor) has unprotected
penetrations in ceiling & walls
* Floor assemblies require fire rated protection -
openings in floor found in water heater room on
2nd floor and in furnace closets on 2nd, 3rd,
and 4th floor.

ceiling to protect penetration (see bedroom in
201)
* Finish rating penetrations in riser room (wall
common to elevator room does not require fire
rating).
All building construction is complete.Sprinkler head in 2nd floor common area is
wrapped in fabric.
