Nephi,
I was going through old permits and it came to my attention that Permit # 16-00672 has not been
paid for, nor have I received an application for it. The job was at 16 South 1st East and it was an
emergency service change-out. The inspection has been completed and passed, but we will need
further documentation.
We understand it was an emergency scenario, however we still need to collect the $65 inspection
fee and an application stating that your business did the work. For your convenience, I have attached
an electrical application. Please send this back to me at your earliest convenience and I will call for
payment.
