To Whom it may concern:
Great Basin Services has removed the asbestos found in the Inspection on this house.
Aprox 300, 3 sq ft of duct tape in the basement, and 1020 sq ft of Transite located on the outside
of the building. All asbestos materials removed, were taken to Bannock County landfill. This is
an EPA approved landfill which accepts Asbestos. A landfill manifest will be given to owner to
show proper disposal as required by EPA.

Introduction
On April-I8-20t2, accredited AHERA Inspector Dave Price of Great Basin Services
preformed an Asbestos inspection on the vacant house ( apartments 125 and 127 South I't West,
Rexburg,ldaho. This inspection was requested by Cory Sorensen. This survey was conducted
to facilitate the future Demolition of this structure. This inspection, as required by EPA, before
demo, will indicate the presence, or lack of, Asbestos Containing Materials (ACM).
Background
The original house is approximately 60-70 years old and has been added on to and
remodeled several times. This house is broke up into 3 separate apartments and has multiple
flooring and finishes on walls and ceilings. Because they look like the have been remodeled at
different times, more sampling was needed . It is aprox 1800 sq ft.. It is currently vacant. It is a
multi- level structure, with a small crawlspace/ basement, approx.300 sq ft. The building has
wood framed walls with some wood, metal and Transite on the outside. On the inside there is
some drywall, and some plaster. The roof has I layer of shingles under newer metal. There is
some fiberous insulation in the ceiling, along with fiberglass in the walls. There was no floor
insulation found. No boiler or piping insulation was found. There was a small amount of duct
tape on ttre heat ducts in the basement.
Sampling
Dave Price, accredited AHERA inspector collected a total of 23 samples of suspect
Asbestos Containing Material (ACM) from both apartment areas. These were broke down into a
total of 44 separate materials, that were tested at the lab. In this sampling scheme, sampling
locations and number of samples were selected so that they were representive of the
homogeneous sampling areas with regard to the material present and it's potential asbestos
content. The sampling scheme was designed to be comprehensive and intended to identiff ACM
that might be disturbed during Demolition/ Renovation activities.
ACM ,under current regulation, is defined as material with an asbestos content l%o or
greater by volume. Any sample analyzed by "polarized light microscopy" ( PLM) method as
containing a trace to less than l0% asbestos must be re-analyzed " by point count method" or
must be assumed, to be ACM.. All samples were shipped to and analyzed by EMSL Analytical,
San Leandro, California. EMSL is a AIHA accredited lab and participate in the NVLAP
program. The materials were sampled for the purpose of identiffing possible Asbestos
containing materials, that would need to be removed, before Demolition of this building. This
survey can be used, to satisff EPA and OSHA regulation requirements for renovation and/or
demolition activities. See drawings for building layout and sample locations.
Conclusions and Recommendations:
The following identifies the material, location, approx. quantity and condition of all materials
identified as containing asbestos. Out of 44 separate materials sampled, 3 samples were found to
contain Asbestos.
Recommendations:
Sample # SV-O1-sheet vinyl in the kitchen of Apartment# 125. This material is also in the
adjacent hall and rooms ( aprox 300 sq ft) This materialtested at20% Chrysotile Asbestos and is
a Friable Regulated material. This material is required to be removed prior to demo, by EPA.
Sample # DT-01 is the duct tape on the heat ducts in the basement of Apartment # 125. This
material tested at30Yo Chrysotile Asbestos and will also need to be removed prior to demo.
Sample # TS-01 is the Transite siding located on the exterior of the apartment # 127, This is the
older portion of the house, with the garage, This material is located on 3 sides of the house. This
material tested at lTYo Ctuysotile asbestos and will need to be removed prior to demo. There is
aprox 1020 sq ft of transite siding.
Asbestos materials that need to be removed are
300 sq ft of sheet vinyl flooring
3 sq ft of duct tape
1020 sq ft of Transite siding
( see attached sheet for info on roofing and floor tile in, Concerns for
owners/demo contractors)
Concerns for Owners and/ or Demolition Contractors:
Roofing : Roofing materials, shingles, built-up roofs, tar, etc., are typically NON-FRIABLE
materials and fall under the National Emissions Standard for Hazardous Air Pollutants ( NESHAP)
regulations found in Appendix A to Subpart M,- Interpretive Rule Goveming Roof Removal
Operations. This rule states that removal of non-Friable roofing materials involving 5580 sq ft or
more is subject to NESHAP regulations, only if equipment that crumbles, chips, or otherwise
renders the roofing materials friable, is used. However some landfills will not accept roofing
materials as general construction debris unless Asbestos testing has been done on that material. If
roofing materials are found to contain Asbestos, some landfills will require to separate any Asbestos
materials and may require packaging or labeling separate from General Construction Debris. Demo
Contractor will need to follow all applicable rules if non-asbestos roofing materials are not removed
prior to demo.
Floor tile: Floor tile is typically NON-FRIABLE and EPA therefore does not require removal prior
to demo by EPA rule. However the same holds true for floor tile as roofing, if it is made to become
friable by crumbling, chipping, sanding, grinding, ect during demo, then this material must be
removed prior to demo. Also landfills may require that any Asbestos materials be separated from
general construction debris, and or packaged differently before disposal. The Demo contractor will
need to follow all applicable rules if non-friable Asbestos floor tile or mastic are not removed prior
to demo.
l. Asbestos containing materials should be removed with trained and certified, Asbestos abatement
workers, using approved methods. The Asbestos material must be packaged, transported, labeled,
and disposed of as required by EPA.
2. A EPA Notification for the Asbestos removal and Demolition portion of this project will need to
be filed, before Abatement and/or Demolition begins. If the asbestos amounts are under the
regulated threshold amount, ( 160 sq ft or 260 lf of Friable Asbestos materials), or are non-friable,
only a courtesy EPA Notification is required.( No l0 day waiting period, before removal of
asbestos). If the asbestos removal amounts are above the regulated threshold amounts, a 10
working day EPA notification is required. This l0 Day notification requires that the abatement/
demolition contractors wait 10 working days, from date of postmark, before abatement or
demolition can begin. The abatement must start on the date stated in this notification. Also the
demolition must start on the date listed as the Demolition start date. Failwe to start on the dates
listed, may constitute warnings and/or fines from EPA. These dates may be changed , if needed, in
writing EPA with a revision. Revisions will need to be sent to EPA, when any changes arise, such
as removal amounts change, dates change, additional removal, date extensions, ect. Also, if a
project is completed before the listed completion date, a revision showing that the project is
complete, must be sent. All revisions, must be sent before the original date of completion expires.
* The Demolition Contractor must pay attention to Section XVil , of the EPA
Notification Of Demolition And Renovation, which states: An individual trained
in the provisions of NESHAPS, (40 CFR Part 61, subpart M) will be on the job-
site during the demolition or renovation, and evidence that the required
training has been accomplished by this person and will be available during
normal business hours.
