Pursuant to the provisions of the Uniform Building Code, the undersigned requests that a
building permit be issued for:

Applewood Apartments - 7 (6 plexes)
347 S. 4` W.
Rexburg, Idaho 83440
Acknowledgment is made that the plans for the complex are not complete and that final
approval of the building will not be given until the final plans have been approved. We
recognize that proceeding with partial construction at this time is entirely at the risk of the
Architect/Owner with there being no assurance that the final Certificate of Occupancy for the
entire building or structure will be granted.
We further absolve the City of Rexburg and officers and employees thereof, of all responsibility
for the issuance of a partial permit and further agree that any work performed under this permit
will be removed or otherwise corrected to be in accord with the requirements of the final
approved plans when a permit for the entire building or structure is finally granted.

I have been performing building inspections on the above named apartments located at
347 South 4 West. As per request of the city.
In review I note that the application for the building permit dated September 17, 2002 #R-
02- 09 -13, it appears this permit was for two building only. Has there been any additional
permits issued? I have been asked to inspect the first two buildings as they have
progressed. Also, I have inspected footings and foundations on five additional buildings.
1 note that the framing is started up to the third floor of an additional building.
If no additional permits have been applied for, the five remaining units are in violation of
the code and have been for some time. Please advise me as to how you want me to
proceed with the inspection work the city has asked that I perform.
