REQUEST FOR A CONDITIONAL BUILDING PERMIT
FOR PARTIAL BUILDING CONSTRUCTION
TO: Building Official for the City of Rexburg
Pursuant to the provisions of the Uniform Building Code, the undersigned requests that a
building permit be issued for:
Footings and Foundations only at:

Rexburg, Idaho 83440
Acknowledgment is made that the plans for the complex are not complete and that final
approval of the building will not be given until the final plans have been approved. We
recognize that proceeding with partial construction at this time is entirely at the risk of the
Architect/Owner with there being no assurance that the final Certificate of Occupancy for the
entire building or structure will be granted.
We further absolve the City of Rexburg and officers and employees thereof, of all responsibility
for the issuance of a partial permit and further agree that any work performed under this permit
will be removed or otherwise corrected to be in accord with the requirements of the final
approved plans when a permit for the entire building or structure is finally granted.

NOTES: 1. Foundation configuration provided by Mike Bressler of SteelVision
Construction.
2. Foundation loads from Behlen Drawings, Sheet 2, Job No. M2795, dated
4/01/03.
3. Exact anchor bolt locations shall be as shown on Behlen Drawings.
4. Concrete 28 day strength shall be 2500 psi minimum. Slump shall be 3-4.5 in.
Reinforcement bars shall be ASTM A615 with F = 40,000 psi.
5. Concrete construction per ACI -318.
6. Allowable soil bearing pressure = 3000 psf
7. Rectangular piers shown on drawings may be replaced with circular piers
formed by sonotubes if the reinforcement is the same, the concrete area is equal
to that of the rectangular pier, and there is sufficient cover for the
reinforcement as required by ACI -318.
