Herewith you will find three -(3) certified copies of the Madison site survey and construction
drawings that you requested on May 5, 2994. These copies were prerequisite, per your request,
before you would issue Verizon Wireless a Building Permit to place a 60 -kw external, stand -by
generator and concrete slab. (Note: The 60 -kw external generator will be equipped with a 210
gallon diesel fuel tank.)
This project was assigned to Harper Leavitt Engineering, LLC. On June 10, 2004, and we
received the finished product on August 19, 2004. Although a long delay, we are in hopes that
we will not encounter any additional delays in receiving our Building Permit on this minor
project.
